The objective of this work is to evaluate the results following implementation of the Plan for Intensification of Malaria Control Actions (PIACM) in endemic municipalities of the Brazilian Amazon Region, in the context of decentralization of health surveillance. This survey investigated the impact of PIACM on both the structure and function of health services, and on malaria incidence in these municipalities. The effectiveness of the plan in reducing malaria morbidity and mortality in municipalities of the Amazon Region was confirmed. The reduction in malariometric indicators was greater in small- and medium-sized municipalities compared to those of larger size. The observed decrease in malaria morbidity was also higher in small- and medium-sized municipalities not certified compared to those certified, contrary to results observed in larger municipalities. PIACM contributed to a change in the dynamics of malaria transmission patterns in the Amazon Region, with a reduction in disease dispersion, resulting in an epidemiological situation more receptive to control actions.
