The study was conducted using different sources of information on hypertensive patients in a Family Health Unit in Londrina, Parana State, Brazil. Three sources were analyzed for ascertaining the number of patients: the System of Enrollment and Monitoring of Hypertensive and Diabetic Patients (Hiperdia), the Primary Health Care Information System (SIAB) and schedule cards. The study aimed also to estimate the rates of under-registration of cases of hypertension as well as to check the reasons for under-registration of patients. Among participants, 689 (71.5%) had not been registered simultaneously in the three sources of information. The main reason for not being registered was that he/she did not live in the area of the Family Health Unit any longer (52.3%). Rates of under-registration ranged from 5.4% (schedule cards) to 39.5% (Hiperdia). The results show a great number of hypertensive patients unregistered simultaneously in the three analyzed sources, what may hinder the adoption of health promotion actions and monitoring of the disease.
