Abstract

OBJECTIVE: to report a clinical case of a chronically infected patient with HBV who developed HCC coming from Brazilian Amazonian. CASE REPORT: female, 46 years old, coming from Medicilandia, country city of Para State, complaining of a painless mass sited at right hypochondrium confirmed by physical examination that demonstrated an enlargement of right hepatic lobe palpable 12 cm below the right costal margin with a hard and irregular surface. The laboratorial finds were normocitic and normocromic anemia and alfa fetal protein more than 480 ng/dl. HBsAg and anti-HBc markers were positives but HBeAg, anti-HD e anti-HCV were negatives. DNA-HBV was lower than 1000 copies/ml. Tomography showed single mass occupying an important area of the right lobe of liver. The surgical intervention was indicated and resection of V, VI and part of VII hepatic segments with a security surgical margin of 5 cm plus cholecystectomy were performed. The histopathological examination diagnosed hepatocellular carcinoma with 13,0 x 12,0 x 11,0 cm in dimensions and liver parenchyma rounding the neoplasm did not show signs of cirrhosis. The hospital stay was 12 days and the patient leaved the hospital presenting good general state. FINAL CONSIDERATIONS: inactive carriers of HBV present higher risk of development of HCC than healthy population and screening with an early diagnosis could make possible curative treatment.
