The 1999 National Tuberculosis Control Plan includes the building of a network of excellence involving the Government, Universities and the Community. A model of this Network was assessed in Rio de Janeiro city - Programme Area 4 (Jacarepagua, Barra da Tijuca and Cidade de Deus). This report describes the results from one of the first trial health units, Unidade Integrada de Saúde Hamilton Land (UISHL), localized in Cidade de Deus. The aim of the programme was to increase the detection rate for tuberculosis in the community, and to reduce treatment non compliance. The authors describe 1) the planning processes involved in setting up this programme and the resources needed, 2) the training of the multidisciplinary teams,3) the implementation of directly observed treatment regimens,4) The domiciliary contact tracing system and the system for identifying those in the community suffering respiratory symptoms,5) the register system,6) the strategies used to involve the community. RESULTS showed that between November 1999 and April 2000 there was a 92% improvement in diagnosis and 83% improvement in the detection of those with respiratory symptoms. The non compliance rate was reduced to zero. The authors conclude that successful control of tuberculosis necessitates involvement of all agencies working together and this methodology could be reproduced in other parts of the country.
