INTRODUCTION: The increase in the elderly population has been accompanied by an increase in the prevalence of tuberculosis, which raises the need for research into this age group. OBJECTIVE: Study the death rate from tuberculosis in all its form in the elderly and in the subgroups, 60 to 69 years and 70 years of age and above, in the state of São Paulo, Brazil, 1940-1995. METHODOLOGY: Mortality coefficients/100,000 inhabitants were calculated on the basis of populational data and of those taken from the death data obtained from the SEADE Foundation, and separated into age groups, distributed in five-year tables by four key periods: prior to chemotherapy; chemotherapy; economic development and Public Health policies; and period of economic crisis and AIDS. RESULTS: Between 1940 and 1995 the mortality coefficient among the elderly diminished less than that of the general population. In the first, second and third periods the elderly maintained considerable reductions which leveled out between 1980 and 1995. In the subgroup of 70 years of age and above the reduction was less than that for persons of 60-69 years of age. DISCUSSION: Among the elderly the coefficients diminished as from 1940-1945 and the reduction was maintained with the help of chemotherapy in the second period and further, benefited by socioeconomic development and Public Health policies in the third period, continued to fall into the first five years, 1980-1985, of the last period. However, as from 1985-1990 the coefficients stagnated due to the economic crisis and the advent of AIDS. CONCLUSION: Mortality from tuberculosis diminished less among the elderly, increasing its participation to a quarter of the total deaths in 1995, the subgroup of 70 years of age and above accounting for half of these; thus this age group is deserving of special attention on the part of those who deal with tuberculosis
