OBJECTIVE: To verify if the introduction of the directly observed therapy influenced the success rates and compare it with the self-administrated therapy. METHODS: Study of cohorts of the completed treatments of tuberculosis on the period from January 1st 1998 to June 30st 1999, in Taboão da Serra city, São Paulo. Besides semestral cohorts from that period, cohorts of the directly observed and self-administrated therapies from September 1st 1998 to July 31st 1999 were also organized. The cohorts were compared according to the success rates. The exclusions that period were considered according to the Almeida’s technique, utilizing on the cases exclusion method used of life tables construction. RESULTS: 198 treatments were selected, 67 cases from January 1st to June 30st 1998 resulted in 80,6% (54) of finalizations. The cohorts from July 31st to December 31st 1998, with 77 treatments, have generated a success rate of 83,1% (64). The cohorts from January 1st to June 30st 1999, with 49 cases, recorded a rate of 89,8% (44) of cure. In the same period, 23 self-administrated therapies were separated with cohorts resulting in 87,0% (20) of success rate. The cohorts of the directly observed therapies from September 1st 1998 to July 31st 1999, with 53 treatments, registered a cure rate of 86,8% (46). The 53 self-administrated therapies in the same period resulted in 83,0% (44) of finalizations. CONCLUSION: The directly observed therapy was more effective. Its introduction promoted a better performance of the tuberculosis control program and have also improved the self-administrated therapy results
