The Fine Chemistry Society for Tropical Diseases (QTROP) has chosen tuberculosis as a target for field work at Duque de Caxias district, RJ. Parameters of care for tuberculosis management in that district are far from optimal: in 1996, the incidlence of tuberculosis was 193/100,000; death rate was 9,28/100,000 in 1995; and cure rate was 65% with 35% of patients defaulting from care. The Vila Rosario region (55,631 inhabitants, very low socio-economics status, tuberculosis case rate under treatment, 103/100,000 and death rate 34,15/100,000) was chosen as a pilot area. From January 2000 onwards, tuberculosis control activities were implemented. The health team consisted of 2 physicians, one nurse, one social assistant and 17 health assistants. Emphasis was on finding new cases and reducing the non compliance rate. During 2000 and 2001,the prevalence of tuberculosis in Vila Rosario was 81 (142/100.000) and 108 (186/100.000), respectively; the rate of case-finding were, 86,35 e 88,11/100.000 during those two years. Examination of the results of a cohort of 140 tuberculosis patients (110 new cases - and 30 retreatment cases) showed that among the new cases, 103 were pulmonary ( 78 smear positive, and 25 smear negative) and 7 were extrapulmonary cases. Treatment results of the 110 new cases treated with 2RHZ/4RH were as follow: favourable: 77,3%; failure: 1,8%; death: 8,2% (5/9 deaths were HIV+). Among the 30 retreatment cases (86,6% treated with 2RHZE/4RH), treatment results were: favourable: 50%; default: 23,3%; failure: 10%; death: 16,7%. Thus improved tuberculosis control activities lead to: 1) reduction of mortality - 21,14/100,000 in 2000 to 10,36 in 2001 (50,9%); 2) favourable results - from 65% to 77,3%; and 3) a reduction in the default rate - from 35% to 8,2%
