Due to the reduced acess to treatment and to efficient drugs, the drug resistance is one of the most important themes nowadays. Beyond the epidemological repercussion, the patient prognostic is private. Global resistance prevalence showed a variation of 5,0% in New Zealand and 42,4% in the Dominican Republic. The revision approaches the mechanism of resistance, the fundamentals in short term chemotherapy, the treatment and the resistance in Brazil, the risk factors for resistance and control program repercussion. The main risk factors for resistance are the unsuitable use, or supply of drugs, late diagnostic, unsuitable acess to treatment, co-infection by T-HIV
