Proper control of tuberculosis leads to reduced human suffering and the social and economic consequences for the ill, his/her family and society. One element of TB control strategy recommended by Brazilian Ministry of Health (BMH) is DOTS strategy (Directly Observed Treatment Short-Course) for all patients with the disease. The supplementary care, as incentives, for patients to promote adherence to treatment must be part in all Tuberculosis Control Program (TCP). Incentives are offered to patients in DOTS to reduce the risks of neglect of treatment, in other words, the patient must adhere to DOTS. Those incentives referred to basic food package during treatment of TB and to transport warranty (transport coupons) to DOTS. This study is an essay and a contribution facilitative of work to the access of incentives for DOTS from TCP’s high burden cities from State of Sao Paulo. This is a descriptive and retrospective study that uses data on supplementary care by Evaluation and Control Instrument (ECI), a questionnaire survey, distributed by BMH with questions about TCP from fi rst semester of 2006. Data received by ECIs correspond to 48 from 73 high burden cities (65.8%). It was observed that 35 cities use DOTS (72.9%). From cities that use DOTS strategy there are cities that offer only one type of incentive (37,1%) and those that offer both incentives, basic food package and transport coupons as supplementary care (45.8%).
