Because of the increasing concern about the adverse effects of air pollution on the populations´ health, it has become necessary to investigate and quantify these effects. A time-series study with the objective of assessing the association between urban levels of air pollution and health effects was conducted in the two biggest Brazilian metropoli: São Paulo and Rio de Janeiro. Daily information on mortality, hospital admissions, air pollution and meteorological variables were obtained for both cities. Data were analyzed using time series techniques in linear models with generalized estimation equations and/or non-parametric models, with generalized additive models. Statistically significant associations between air pollution levels and mortality and hospital admissions for respiratory and cardiovascular causes, for children and the elderly, were found in both cities. These associations remained after adjustment for long term trends, seasonality, temperature and humidity. The current air pollution levels in São Paulo and Rio de Janeiro are capable of producing harmful effects in the health of the population. Articulated measures by those who manage urban life in the metropolis are fundamental to improve air quality in both cities and thereby improve the population´s health
