The dynamics of dengue virus circulation in intra-urban areas of large metropolis and the risk factors for infection are still not well understood. Despite poverty being a recognized determinant for most infectious and parasitic diseases, it continues to be controversial for dengue. The objectives of this study were: to describe the seroprevalence and seroincidence of dengue in different intra-urban areas of a large complex city in Northeastern Brazil, and to assess the relationship between viral circulation intensity and life conditions of the study population, as well as vectorial infestation. A prospective study with ecological analyses was designed. Sorological surveys were conducted in a sample of the population resident in 30 different areas of the city of Salvador - “sentinel areas” - selected according to different social-economic status. The results revealed high seroprevalence (67,7%) and incidence of infection (70,6%) for the circulating serotypes DEN-1 and DEN-2, with great variability of the rate estimates in the 30 study areas, with low effectiveness of vector control measures. Viral circulation was present in all the territory of the city and adequate living conditions were unable to limit the ocurrence of dengue transmission.
