The Family Health Care Program (PSF) has been advertised as the main strategy for reorganizing Brazil´s new National model for health care. However, due to the actual administration and local organizational systems, very unique experiences of PSF implantation are observed. From this perspective, the current article analyzes the PSF experience by observing two municipalities in Bahia State, Brazil. Descriptions of the organization and administration of the Program, practices profile and coverage served to discuss three different possible scenarios on the format of the PSF model: incipient; intermediate; and advanced. The difficulty articulating between different levels of the system, the financing and the precarious living conditions appear as obstacles to the Program’s development. On the other hand, characteristics such as the municipal management´s capacity to prioritize the Program attitude and the commitment of the groups associated with a new model of participatory administration are factors that have a positive influence on the building of this proposal.
