OBJECTIVE: tthe aim of this study was to analyze the trends in lung cancer mortality by region in the State of Pernambuco, Brazil, between 1996 and 2005. METHODOLOGY: the study population was individuals above 50 years old living in Pernambuco who passed away in the study period; it was a time series study with data from the Brazilian Mortality Database (SIM), available at the Pernambuco Health Secretariat; raw and age standardized mortality rates were calculated; trends were analyzed by sex and age, using simple linear regression. RESULTS: the Metropolitan Region of Recife, capital of the State of Pernambuco, had the highest number of death (2,594), as well as the highest mortality rates, ranging from 52.2 per 100,000 population in 1996 to 59.4 per 100,000 population in 2005; the analysis by sex showed an upward trend for the State of Pernambuco (p=0,005) and thefollowing regions of Agreste Pernambucano (p<0.0001), Mata Pernambucana (p<0.001) and Sao Francisco Pernambucano (p<0.005). CONCLUSION: the study recommends that effective public health measures for tobacco control in Pernambuco should consider gender and regional differences.
