OBJECTIVE: this study analyzed intraurban differences in biological, socioeconomic and care factors related to inherited syphilis in the Municipality of Recife, State of Pernambuco, Brazil, between 2004 and 2006. METHODOLOGY: a descriptive ecological study based on data from the Information System for Notifiable Diseases (Sinan) and the Newborn Information System (Sinasc); city districts were classified in relation to living conditions and to the proportion of live births characterized by low birth weight, black colored, children from mothers aged less than 20years, schooling less than 4 years, gestational age less than 37 weeks, and mothers who had less than four prenatal consultations. RESULTS: seven hundred cases of inherited syphilis were detected (10.2/1,000 live births); after comparing data from three district strata, the study identified as risk factors, black people or people with mixed color/race, living conditions, mothers aged less than 20 years, schooling less than 4 years and prenatal consultations less than 4, along with the associated consequence of low birth weight. CONCLUSION: inherited syphilis presented uneven distribution in the urban space, thus requiring differential intervention strategies.
