OBJECTIVE: the study aims to evaluate the impact of death cause investigation related to poorly defined causes on mortality profile by groups of causes in the State of Piauí, Brazil. METHODOLOGY: Mortality Information System data refereed to the period from 2000 to 2006was used; the study, retrospective and ecological, was divided into two stages: a) analysis of temporal evolution of proportional mortality due to poorly defined causes (PMPDC) before and after intervention; and b) comparison of mortality profile by groups of causes, before and after recording of causes. RESULTS: PMPDC decreased 28.2% from 2000 to 2005 (before intervention), and 67.3% from 2005 to 2006; distribution profile of deaths by groups of causes continued unaltered. CONCLUSION: the methodology of death investigation provided information about mortality; the shortage death verification services along with resistance of physicians to issue death certificates are the main difficulties for elucidation of death causes.
