<body><![CDATA[<p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><a href="#nota">Endere&ccedil;o    para correspond&ecirc;ncia</a></font></p>     <p>&nbsp;</p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> O sistema MB/BacT    desenvolvido pela Organon Teknika para o isolamento de micobact&eacute;rias    &eacute; totalmente automatizado e n&atilde;o radiom&eacute;trico. Baseado no    convencional meio 7H9 de Middlebrook, utiliza um sensor colorim&eacute;trico    para detec&ccedil;&atilde;o de CO<sub>2</sub> produzido pelo metabolismo das    c&eacute;lulas. Em rela&ccedil;&atilde;o a descontamina&ccedil;&atilde;o dos    materiais cl&iacute;nicos para o isolamento de micobact&eacute;rias, o fabricante    recomenda o m&eacute;todo da n-acetil ciste&iacute;na (NALC) com hidr&oacute;xido    de s&oacute;dio. De 1996 at&eacute; o momento v&aacute;rios autores apresentaram    resultados de compara&ccedil;&otilde;es entre esta metodologia com outras mais    tradicionais, em termos de percentual de isolamento, contamina&ccedil;&atilde;o,    tempo de crescimento utilizando o NALC <i><sup>(1-5)</sup></i> . Carricajo <b>et    al</b>. (2001) utilizaram lauryl sulfato de s&oacute;dio como agente descontaminante,    com bons resultados neste sistema. O objetivo deste trabalho foi avaliar o fosfato    triss&oacute;dico como agente descontaminante para o m&eacute;todo automatizado    MB/BacT. Foram examinadas 346 amostras de escarro de pacientes com suspeita    clinica de tuberculose, da nossa rotina de laborat&oacute;rio de refer&ecirc;ncia,    no ano de 1999. Os escarros foram descontaminados pelo M&eacute;todo de Corper    &amp; Stoner modificado, que utiliza fosfato triss&oacute;dico a 23% como agente    descontaminante em contato com o mesmo volume de escarro durante 24 horas e    fosfato monoss&oacute;dico a 20% como agente neutralizante vol/vol. O material    tratado foi semeado em tubos com meio de Lowenstein Jensen - LJ (padr&atilde;o    ouro) e em frascos com meio de cultura do sistema MB/BacT. Os frascos desse    sistema foram monitorados a cada 10 minutos, quanto ao crescimento, e as culturas    em LJ, semanalmente. A identifica&ccedil;&atilde;o de <i>M. tuberculosis</i> foi feita    atrav&eacute;s do crescimento em PNB e TCH (agentes inibidores). O resultado    final apresentados pelas semeaduras em ambos os meios foi registrado ap&oacute;s    6 semanas de incuba&ccedil;&atilde;o a 35-37<sup>o</sup>C.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Os dados obtidos,    apresentados na tabela abaixo, permitem avaliar o desempenho do m&eacute;todo    em termos de: sensibilidade - 93% ( 86,5 - 97,1 ), especificidade - 97,5% (    94,4 - 99,0) e de valores preditivos de positividade enegatividade - 94% ( 87,6    - 97,7) e 97,11% (93,8 - 98,7) respectivamente, com o intervalo de confian&ccedil;a    de 95%. O tempo m&eacute;dio para detec&ccedil;&atilde;o no MB/BacT foi de 19    dias e nenhuma contamina&ccedil;&atilde;o foi observada neste experimento, contrastando    com experimentos preliminares usando o m&eacute;todo do NALC onde a contamina&ccedil;&atilde;o    foi muito al&eacute;m daquela tolerada ( &gt; 5%). Estes dados mostram que a    metodologia de descontamina&ccedil;&atilde;o empregada neste sistema n&atilde;o    causou a inibi&ccedil;&atilde;o do crescimento das c&eacute;lulas, permitindo    um bom isolamento.
