<body><![CDATA[<p>&nbsp;</p>     <p>&nbsp;</p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> <b>SUMMARY</b></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> <b>BACKGROUND:</b>    Mortality and morbidity differences between social classes have been observed    in almost all countries in the world. In order to monitor the effect and quality    of health services, mortality caused by avoidable causes have been used as negative    indicators or &#8220;sentinel events&#8221;. The analysis of the impact of public    policies on the population&#8217;s health outcomes can be made through the study    of avoidable deaths combined with that of socioeconomic inequalities. An indicator    that measures the risk of death attributed to life conditions known as the Mortality    Reducible Gaps (MRG) was developed by the Pan American Health Organization (PAHO).    This MRG indicator also permits the evaluation of the effectiveness of health    services by comparing mortality due to avoidable causes between regions with    similar social characteristics. In Brazil, this approach has not been explored    well to date. This paper has the objective of comparing trends in mortality    gaps of avoidable deaths inside clusters of urban settings with similar living    conditions during the period between 1980 to 1998.     <br>   <b>MATERIAL AND METHODS:</b> A time series study of selected avoidable deaths    in Brazilian state capital cities from 1980 to 1998 was carried out. Standardized    mortality rates were estimated for tuberculosis and cerebro-vascular disease    as well as, diarrhea and acute respiratory infection related mortality rates    in infants. Cities were stratified by life conditions using principal component    analysis. For each of the urban settings analysed, stratified (SRM) and overall    Reducible Mortality Gap (RMG), was calculated, and defined as the difference    that can be avoided by comparison to a standard measure. Velocity and percentage    of reduction, and inequality ratios between urban settings were also calculated.    <br>   <b>RESULTS:</b> Decreasing trends in infant diarrheal mortality rates (between    &#8211;5,3% and &#8211;77,4%) and for acute respiratory infection (between &#8211;    7,1% and 69,9%) were observed in most of the urban settings and for all the    period studied. Despite the decline in the majority of the urban settings studied    for tuberculosis and cerebro-vascular mortality, there was an increasing trend    in several areas. Tuberculosis SRM showed variable patterns along the time series    studied and between urban settings. Cerebro-vascular mortality SRM had a decreasing    pattern during the two decades. However, the overall values were high for both    causes of death especially in the stratum of lowest life conditions. Inequality    ratios were also high and ranged from 0,4 to 11,0.    <br>   <b>CONCLUSIONS:</b> Deacreasing trends of SRM for both diarrhea and acute respiratory    infection in infancy are possible effects of specific programs directed to the    control of these health events. The unstable pattern and increasing trend for    tuberculosis and cerebrovascular mortality indicate that strategies of control    measures are not effective. <body><![CDATA[<p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b>DELINEAMENTO    DO PROBLEMA: </b>Diferen&ccedil;as na mortalidade e na morbidade entre estratos    e classes sociais t&ecirc;m sido observadas em praticamente todos os pa&iacute;ses    do mundo. Por outro lado, medidas de mortalidade por causas evit&aacute;veis    t&ecirc;m sido utilizadas como indicadores negativos, &#8220;eventos sentinela&#8221;,    capazes de monitorar o efeito e a qualidade dos servi&ccedil;os de sa&uacute;de.    A an&aacute;lise do impacto de pol&iacute;ticas p&uacute;blicas sobre o estado    de sa&uacute;de de popula&ccedil;&otilde;es pode ser feita mediante articula&ccedil;&atilde;o    entre o estudo das mortes consideradas como evit&aacute;veis com aquele referente    aos diferenciais socioecon&ocirc;micos das popula&ccedil;&otilde;es. Nessa perspectiva,    a Organiza&ccedil;&atilde;o Panamericana da Sa&uacute;de (OPAS) desenvolveu    um indicador denominado <i> Brechas Redut&iacute;veis de Mortalidade  </i>(BRM) que mede    o risco de morte atribu&iacute;vel &agrave;s condi&ccedil;&otilde;es de vida.    Esse indicador pode, contudo, tamb&eacute;m revelar indiretamente a efetividade    dos servi&ccedil;os de sa&uacute;de quando se compara o comportamento da mortalidade    por causas evit&aacute;veis entre regi&otilde;es com caracter&iacute;sticas    sociais semelhantes. Tendo em vista que, no Brasil, essa abordagem tem sido    pouco explorada, o presente estudo tem como objetivo comparar a evolu&ccedil;&atilde;o    dos diferenciais de mortalidade por causas evit&aacute;veis selecionadas ao    interior de grupos de capitais brasileiras, com condi&ccedil;&otilde;es de vida    semelhantes, no per&iacute;odo compreendido entre 1980 e 1998.     <br>   <b>MATERIAL E M&Eacute;TODOS:</b> Foi realizado estudo de s&eacute;ries temporais    de causas selecionadas de mortalidade evit&aacute;vel, no per&iacute;odo compreendido    entre 1980 e 1998, nas capitais brasileiras. Foram estimados os coeficientes    de mortalidade padronizados para tuberculose e doen&ccedil;a cerebrovascular,    al&eacute;m dos coeficientes de mortalidade infantil por diarr&eacute;ia e infec&ccedil;&atilde;o    respirat&oacute;ria aguda. A estratifica&ccedil;&atilde;o das capitais segundo    condi&ccedil;&otilde;es de vida foi feita por meio da t&eacute;cnica estat&iacute;stica    da an&aacute;lise de componentes principais. Para cada uma das capitais foram    calculadas as <i> Brechas Redut&iacute;veis de Mortalidade  </i>(BRM) em rela&ccedil;&atilde;o    ao estrato (BRE) e ao pa&iacute;s (BRP), a velocidade e o percentual da redu&ccedil;&atilde;o    al&eacute;m de  <i>raz&atilde;o de desigualdade </i> entre as capitais. As  <i>Brechas Redut&iacute;veis    de Mortalidade </i> (BRM) s&atilde;o definidas como diferenciais que podem ser reduzidos    em rela&ccedil;&atilde;o a um padr&atilde;o de compara&ccedil;&atilde;o.    <br>   <b>RESULTADOS:</b> Verificou-se tend&ecirc;ncia majorit&aacute;ria de decr&eacute;scimo    da mortalidade infantil em todo per&iacute;odo estudado, na maioria das capitais    brasileiras, tanto por diarr&eacute;ia (entre &#8211;5,3 e &#8211; 77,4%) como    pelas infec&ccedil;&otilde;es respirat&oacute;rias agudas (entre &#8211; 7,    1% e 69,9%). Observouse tamb&eacute;m redu&ccedil;&atilde;o do valor m&eacute;dio    das BRE, para ambas as causas em todo o per&iacute;odo estudado. Em rela&ccedil;&atilde;o    &agrave; mortalidade por tuberculose e doen&ccedil;a cerebrovascular, embora    houvesse ocorrido redu&ccedil;&atilde;o das m&eacute;dias q&uuml;inq&uuml;enais,    na maioria das capitais, verificouse aumento em diversas delas. Em rela&ccedil;&atilde;o    &agrave;s BRE, observou-se comportamento vari&aacute;vel, nos diversos q&uuml;inq&uuml;&ecirc;nios,    no que diz respeito &agrave; tuberculose. Em rela&ccedil;&atilde;o &agrave;    doen&ccedil;a cerebrovascular, as BRE apresentaram redu&ccedil;&atilde;o para    todo o per&iacute;odo estudado. Os valores das BRP foram elevados, para ambas    as causas, particularmente no estrato de piores condi&ccedil;&otilde;es de vida.    Tamb&eacute;m foram elevados os valores referentes &agrave;s  <i>raz&otilde;es de    desigualdade </i> que variaram entre 0,4 e 11,0.    <br>   <b>CONCLUS&Otilde;ES:</b> A redu&ccedil;&atilde;o do valor m&eacute;dio das    BRE para a mortalidade infantil, tanto por diarr&eacute;ia como por infec&ccedil;&otilde;es    respirat&oacute;rias, revela poss&iacute;vel efeito dos programas espec&iacute;ficos    voltados para o controle desses agravos. J&aacute; o comportamento vari&aacute;vel    e mesmo aumento verificado em rela&ccedil;&atilde;o &agrave;s doen&ccedil;as    cerebrovasculares e &agrave; tuberculose, em diversas capitais, revela problemas    nas estrat&eacute;gias de controle desses agravos.
