Salmonella spp., Aeromonas spp. e Plesiomonas shigelloides também foram detectados. Os sorotipos de Shigella mais frequentemente encontrados foram S. flexneri (61,1%) e S. sonnei (38,9%). Entre as Salmonella, identificaram-se os seguintes sorotipos: S. Panama (2), S. Newport (2), S. Bredeney, S. Saintpaul e S. Gaminara. A infecção pelos protozoários alcançou 85,3% tendo como principais responsáveis, por ordem de frequência, Blastocystis hominis (37,3%), Entamoeba histolytica/E. dispar (22,8%), Endolimax nana (16,7%), Entamoeba coli (9,5%) e Giardia lamblia (8,4%). Esses resultados fornecem valiosos subsídios à vigilância epidemiológica e ambiental, demonstrando as condições insatisfatórias de saneamento básico em que vive a população do Município.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[With the aim of determining the frequency rate of enteropathogenic bacteria and enteroparasites in patients with acute diarrhea in the Municipality of Juruti, Pará State, which has been facing an intense migratory process due to a great mining project, we analyzed 263 cases of acute diarrhea. A total of 19% of those cases were related to enteropathogenic bacteria, particularly Shigella spp. that accounted for 13.7% of the cases. Salmonella spp., Aeromonas spp. and Plesiomonas shigeloide were also detected. The most frequent Shigella serotypes found were S. flexneri (61.1%) and S. sonnei (38.9%). Among the Salmonella, we could observe the following serotypes: S. Panama (2), S. Newport (2), S. Bredeney, S. Saintpaul and S. Gaminara. The infection caused by protozoa reached 85.3%, mostly due to Blastocystis hominis (37.3%), Entamoeba histolytica/E. dispar (22.8%), Endolimax nana (16.7%), Entamoeba coli (9.5%) and Giardia lamblia (8.4%). These results provide valuable data to epidemiological and environmental surveillance because it presents the unsatisfactory basic sanitary conditions of the local population.]]></p></abstract>
<abstract abstract-type="short" xml:lang="es"><p><![CDATA[Con el objetivo de determinar la frecuencia de bacterias enteropatógenas y de enteroparásitos en pacientes con diarrea aguda en el Municipio de Juruti, Pará, que pasa por intenso proceso migratorio delante de un gran proyecto de explotación mineral, se evaluaron 263 casos de diarrea aguda, de los cuales un 19% fueron atribuidos a bacterias enteropatogénas, destacando la Shigella spp., que representó un 13,7%. También fueron detectadas Salmonella spp., Aeromonas spp. y Plesiomonas shigeloide. Los serotipos de Shigella más frecuentemente encontrados fueron S. flexneri (61,1%) y S. sonnei (38,9%). Entre las Salmonella, se identificaron los siguientes serotipos: S. Panama (2), S. Newport (2), S. Bredeney, S. Saintpaul y S. Gaminara. La infección por protozoarios alcanzó un 85,3% teniendo como principales responsables, por orden de frecuencia, a Blastocystis hominis (37,3%), Entamoeba histolytica/E. dispar (22,8%), Endolimax nana (16,7%), Entamoeba coli (9,5%) y Giardia lamblia (8,4%). O desmame precoce, o baixo n&iacute;vel   educacional e as m&aacute;s condi&ccedil;&otilde;es de saneamento ambiental   s&atilde;o fatores predisponentes para a doen&ccedil;a diarreica<sup>33</sup>.</font></p>     <p><font size="2" face="Verdana">Dentre as causas infecciosas de diarreia incluem-se v&iacute;rus, bact&eacute;rias   e protozo&aacute;rios, sendo os principais agentes   etiol&oacute;gicos: rotav&iacute;rus, caliciv&iacute;rus, astrov&iacute;rus, <i>Escherichia</i>   <i>coli </i>diarreiog&ecirc;nicas, <i>Shigella </i>spp., <i>Salmonella </i>spp.,   <i>Entamoeba histolytica</i>, <i>Giardia lamblia</i>, <i>Cryptosporidiumparvum </i>e <i>Cyclospora     cayetanensis</i><sup>10,22,28,29,32</sup>.</font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana">No Brasil, as infec&ccedil;&otilde;es intestinais     t&ecirc;m     merecido destaque, especialmente aquelas causadas pelas <i>E. coli</i>    diarreiog&ecirc;nicas<sup>2,4</sup>,   por rotav&iacute;rus<sup>15,19</sup>, por <i>Cryptosporidium </i>e  <i>Entamoeba histolytica</i><sup>21,25</sup>.</font></p>     <p><font size="2" face="Verdana">A maioria das diarreias bacterianas requer medidas     de interven&ccedil;&atilde;o voltadas apenas &agrave; aplica&ccedil;&atilde;o   da corre&ccedil;&atilde;o de   sintomas e do desequil&iacute;brio hidroeletrol&iacute;tico. Por&eacute;m,   vale   ressaltar que um caso de diarreia pode suscitar medidas de   vigil&acirc;ncia epidemiol&oacute;gica, face &agrave; possibilidade do   envolvimento de agentes potencialmente epid&ecirc;micos    como o <i>V. cholerae </i>O1<sup>27</sup>, al&eacute;m de nortear medidas   terap&ecirc;uticas para interromper a cadeia de transmiss&atilde;o,   como na c&oacute;lera, ou para reduzir ou evitar os riscos de   complica&ccedil;&otilde;es determinadas por bact&eacute;rias virulentas como   a <i>Shigella </i>e <i>S</i>. Typhi<sup>6,17</sup>.</font></p>     <p><font size="2" face="Verdana">Algumas enteroparasitoses t&ecirc;m import&acirc;ncia    relevante na patog&ecirc;nese da diarreia, e v&aacute;rios estudos t&ecirc;m    associado a <i>Giardia lamblia</i>, o <i>Crypstosporidium </i>sp. e a <i>Entamoeba    histolytica </i>com a doen&ccedil;a<sup>21,25,30,3</sup>. A transmiss&atilde;o    das parasitoses intestinais se estabelece de maneira f&aacute;cil e cont&iacute;nua,    principalmente nos pa&iacute;ses em desenvolvimento, onde o crescimento populacional    n&atilde;o &eacute; acompanhado da melhoria das condi&ccedil;&otilde;es de vida    da popula&ccedil;&atilde;o<sup>26</sup>, tornando-se um problema permanente    de sa&uacute;de p&uacute;blica.</font></p>     <p><font size="2" face="Verdana">Informa&ccedil;&otilde;es da Secretaria Municipal     de Sa&uacute;de   (SEMSA) de Juruti, Par&aacute;, d&atilde;o conta que a diarreia constitui   um dos principais agravos da popula&ccedil;&atilde;o, devido &agrave;  falta   de   infraestrutura no saneamento b&aacute;sico e no abastecimento    de &aacute;gua pot&aacute;vel<sup>23</sup>. Ali pouco se conhece sobre as causas   da doen&ccedil;a diarreica, devido &agrave; car&ecirc;ncia de pessoal t&eacute;cnico   qualificado e dos m&eacute;todos adequados de diagn&oacute;stico.   Assim, o objetivo do estudo foi conhecer a rela&ccedil;&atilde;o dos   enteropat&oacute;genos bacterianos e parasit&aacute;rios nas diarreias   agudas no Munic&iacute;pio de Juriti, Par&aacute;.</font></p>     <p>&nbsp;</p>     <p><b><font size="3" face="Verdana">MATERIAIS E M&Eacute;TODOS</font></b></p>     <p><font size="2" face="Verdana">Foram selecionados para o estudo 332 pacientes,   sendo 286 (86%) provenientes da &aacute;rea urbana e os demais   da rural. Do total, 263 pacientes apresentavam diarreia   aguda (casos: tr&ecirc;s ou mais evacua&ccedil;&otilde;es/dia) e 69 n&atilde;o   tinham diarreia (controles: aus&ecirc;ncia de diarreia nos   &uacute;ltimos 15 dias). Os sujeitos foram constitu&iacute;dos de   crian&ccedil;as e adultos atendidos em tr&ecirc;s unidades de sa&uacute;de   do   Munic&iacute;pio de Juruti. Os pacientes ou seus respons&aacute;veis   assinaram um <span style="font-size:10.0pt;font-family:Verdana;mso-ansi-language: PT-BR">termo de consentimento livre e esclarecido</span>  autorizando sua participa&ccedil;&atilde;o no estudo. O presente   trabalho, sob protocolo n<sup>o</sup> 030/05, CAAE:   0013.0.072.000-06, foi aprovado pelo Comit&ecirc; de  &Eacute;tica em Pesquisa do Instituto Evandro Chagas (IEC). </font></p>     <p><font size="2" face="Verdana">O exame parasitol&oacute;gico foi procedido     sob microscopia de luz para pesquisa de formas evolutivas de protozo&aacute;rios   e helmintos, utilizando os m&eacute;todos diretos (salina/Lugol) e   sedimenta&ccedil;&atilde;o espont&acirc;nea<sup>5</sup>.</font></p>     <p><font size="2" face="Verdana">Para o isolamento de bact&eacute;rias enteropatog&ecirc;nicas,    o material fecal em suspens&atilde;o foi semeado em meios seletivos indicadores    (&aacute;gar <i>Mac Conkey </i>&ndash; MC, &aacute;gar <i>Salmonella</i>-<i>Shigella    </i>&ndash; SS, &aacute;gar Tiossulfato Citrato Bile Sacarose &ndash; TCBS)    e em meios de enriquecimento (Selenito Cistina e &Aacute;gua Peptonada Alcalina    &ndash; APA, pH 8,5). As col&ocirc;nias suspeitas foram submetidas aos meios    de triagem: <i>Triple sugar iron </i>&ndash;TSI e <i>Kligler iron &aacute;gar    </i>(KIA). As caracteriza&ccedil;&otilde;es bioqu&iacute;micas foram realizadas    pelo m&eacute;todo convencional e Sistema API-20E (bioM&eacute;rieux)<sup>14</sup>    e as sorol&oacute;gicas segundo recomenda&ccedil;&otilde;es de Ewing<sup>8</sup>    e Kauffmann<sup>11</sup>.</font></p>     <p><font size="2" face="Verdana">Categorias patog&ecirc;nicas de <i>Escherichia    coli</i>: EPEC-<i>E. coli</i> enteropatog&ecirc;nica, ETEC-<i>E. coli </i>enterotoxig&ecirc;nica,    STEC-<i>E.</i> <i>coli </i>produtora de toxina de <i>Shiga </i>e EIEC-<i>E.    coli</i> enteroinvasora, foram pesquisadas por meio da determina&ccedil;&atilde;o    de genes de virul&ecirc;ncia, utilizando-se <i>primers</i> espec&iacute;ficos    de cada fator de virul&ecirc;ncia para realiza&ccedil;&atilde;o da rea&ccedil;&atilde;o    em cadeia mediada pela polimerase (PCR)<sup>7,9,18,20,31,34</sup>.</font></p>     ]]></body>
<body><![CDATA[<p>&nbsp;</p>     <p><b><font size="3" face="Verdana">RESULTADOS</font></b></p>     <p><font size="2" face="Verdana">Os resultados est&atilde;o apresentados na <a href="#t1">tabela       1</a>,   mostrando que, dos 263 casos de diarreia aguda, 50 (19%)   apresentavam enteropat&oacute;genos bacterianos. Destes, 36   (13,7%) s&atilde;o representados por <i>Shigella </i>spp., sete (2,7%) por   <i>Salmonella </i>spp. 6 (2,2%) por <i>Aeromonas </i>spp. e 1   (0,4%) de <i>P. shigelloides</i>. Considerando as infec&ccedil;&otilde;es pelos   parasitas, conv&eacute;m destacar a associa&ccedil;&atilde;o entre dois ou   mais   parasitas em 88 pacientes (33,5%), em rela&ccedil;&atilde;o aos 74   monoparasitados (28,2%). Entre os parasitas se destacam <i>B. hominis</i>, <i>E. histolytica</i>/<i>E. dispar</i>, <i>E. nana</i>, <i>Ancilostomideo</i>   e <i>A. lumbricoides</i>. Parasitismo por <i>G. lamblia </i>foi   encontrado em 22 (8,4%) dos casos. Dentre as bact&eacute;rias   enteropatog&ecirc;nicas foi verificada associa&ccedil;&atilde;o em apenas   uma ocasi&atilde;o, <i>S. flexneri </i>e <i>A. sobria</i>. O sorotipo de <i>Shigella</i>   mais frequente foi representado por <i>S. flexneri </i>(61,1%)   seguido de <i>S. sonnei </i>(38,9%). Entre as <i>Salmonella</i>,   identificaram-se os seguintes sorotipos: <i>S</i>. Panama (2), <i>S</i>.   Newport (2), <i>S</i>. Bredeney, <i>S</i>. Saintpaul e <i>S</i>. Gaminara.   At&eacute; o   momento pesquisaram-se categorias patog&ecirc;ncias de <i>Escherichia coli </i>em 35 pacientes com diarreia aguda.   Destes, oito estavam infectados com <i>E. coli </i>diarreiog&ecirc;ncias,   sendo sete por <i>E. coli </i>enterotoxig&ecirc;nica - ETEC (LT + &#091;3&#093; e Sta   &#091;4&#093;) e uma por <i>E. coli </i>enteropatog&ecirc;nica - EPEC at&iacute;pica.</font></p>     <p><a name="t1"></a></p>     <p>&nbsp;</p>     <p align="center"><img src="/img/revistas/rpas/v1n1/1a20t1.gif" border="0"></p>     <p>&nbsp;</p>     <p><font size="2" face="Verdana">Quando comparamos os resultados entre diarreicos    e os controles, observa-se que a frequ&ecirc;ncia de agentes bacterianos &eacute;    maior nos diarreicos, com destaque para o g&ecirc;nero <i>Shigella </i>spp.    Dentre os parasitas, esta diferen&ccedil;a n&atilde;o foi observada; no entanto,    destaca-se a frequ&ecirc;ncia de <i>E. histolytica/E. dispar </i>e <i>G. lamblia    </i>(<a href="#t2">Tabela 2</a>).</font></p>     <p><a name="t2" id="t2"></a></p>     <p>&nbsp;</p>     ]]></body>
<body><![CDATA[<p align="center"><img src="/img/revistas/rpas/v1n1/1a20t2.gif" border="0"></p>     <p>&nbsp;</p>     <p><b><font size="3" face="Verdana">DISCUSS&Atilde;O E CONCLUS&Atilde;O</font></b></p>     <p><font size="2" face="Verdana">No c&ocirc;mputo geral, bact&eacute;rias enteropatog&ecirc;nicas   representaram 19% dos casos de diarreia avaliados,   sendo o g&ecirc;nero <i>Shigella </i>o mais frequente, com destaque   para <i>S. flexneri </i>e <i>S. sonnei, </i>que t&ecirc;m sido as esp&eacute;cies   mais<i> </i>frequentemente detectadas no Brasil<sup>2</sup>. Dos pat&oacute;genos    bacterianos considerados emergentes<sup>16</sup>, <i>Aeromonas </i>e   <i>Plesiomonas </i>foram identificados entre os casos de diarreia   aguda. Cinco sorovares de <i>Salmonella </i>foram identificados   dentre os casos diarreicos e nenhum caso de <i>S. </i>Typhi ou   <i>Vibrio </i>foi encontrado. Mesmo assim, conv&eacute;m destacar a   import&acirc;ncia da pesquisa de <i>S. </i>Typhi, agente da febre   tifoide, doen&ccedil;a de notifica&ccedil;&atilde;o compuls&oacute;ria respons&aacute;vel por epidemias em diversos munic&iacute;pios do Estado do Par&aacute;<sup>24</sup>. </font></p>     <p><font size="2" face="Verdana">A import&acirc;ncia das <i>E. coli </i>&quot;diarreiog&ecirc;nicas&quot;    nos casos diarreicos tem sido demonstrada por diferentes autores<sup>28,2,19</sup>,    e a evid&ecirc;ncia de sua ocorr&ecirc;ncia no Estado do Par&aacute;, conforme    constatado nesse estudo, mostra sua ampla distribui&ccedil;&atilde;o e consolida    sua import&acirc;ncia em sa&uacute;de p&uacute;blica.</font></p>     <p><font size="2" face="Verdana">Verifica-se uma importante participa&ccedil;&atilde;o    dos parasitas nos processos ent&eacute;ricos na popula&ccedil;&atilde;o de Juruti.    Grande diversidade de protozo&aacute;rios e helmintos foi observada entre os    pacientes com diarreia, sendo a <i>E. histolytica/E.</i> <i>dispar </i>e <i>B.    hominis </i>os mais frequentes, apesar deste &uacute;ltimo ser atualmente de    patogenicidade discutida<sup>1</sup>. No Brasil, a participa&ccedil;&atilde;o    dos protozo&aacute;rios como agentes respons&aacute;veis por diarreias agudas    tem sido relatada por alguns autores<sup>25,30</sup>. Como esperado, a faixa    et&aacute;ria de 1 a 4 anos teve maior n&uacute;mero de indiv&iacute;duos parasitados,    seguida por aqueles com idade entre 5 e 10 anos, corroborando os dados da literatura<sup>13</sup>.</font></p>     <p><font size="2" face="Verdana">Os resultados encontrados fornecem subs&iacute;dios   importantes &agrave; vigil&acirc;ncia epidemiol&oacute;gica e ambiental,   demonstrando que a popula&ccedil;&atilde;o se encontra sob potencial   risco de desenvolver doen&ccedil;a diarreica em fun&ccedil;&atilde;o das   prec&aacute;rias condi&ccedil;&otilde;es de saneamento b&aacute;sico em que   vive.</font></p>     <p><font size="2" face="Verdana">A continuidade das avalia&ccedil;&otilde;es deste estudo,   associada &agrave;s futuras an&aacute;lises moleculares para pesquisa   de categorias patog&ecirc;nicas de <i>E. coli</i>, &agrave; pesquisa de   rotavirus, bem como &agrave; pesquisa de ant&iacute;genos para <i>G.</i>   <i>lamblia</i>, <i>E. histolytica </i>e <i>Cryptopsoridium </i>sp. poder&atilde;o   oferecer dados reais da participa&ccedil;&atilde;o destes pat&oacute;genos   na   etiologia das doen&ccedil;as diarreicas nesse Munic&iacute;pio.</font></p>     <p><font size="2" face="Verdana">Ressalte-se que a presen&ccedil;a marcante de   enteropat&oacute;genos na popula&ccedil;&atilde;o de Juruti, Par&aacute;, inclusive   com a ocorr&ecirc;ncia de agentes emergentes, somada ao   elevado movimento imigrat&oacute;rio face &agrave; implanta&ccedil;&atilde;o   de   projeto de grande envergadura, que ora ocorre na regi&atilde;o,   denota a necessidade de se manter um servi&ccedil;o permanente   de vigil&acirc;ncia no tocante aos agentes causadores de   diarreia, sobretudo os bacterianos, inclusive pelo elevado   potencial epid&ecirc;mico de alguns deles.</font></p>     <p>&nbsp;</p>     ]]></body>
