Para sua correta identificação, esses microrganismos devem ser diferenciados dos membros não patogênicos da microbiota intestinal. As DEC podem ser classificadas em seis categorias, de acordo com seu mecanismo de patogenicidade. MATERIAIS E MÉTODOS: Foram desenvolvidos e avaliados dois sistemas de PCR multiplex para a detecção de E. coli enteropatogênicas (EPEC), enterotoxigênica (ETEC), enteroinvasora (EIEC) e produtora da toxina de Shiga (STEC). RESULTADOS: Quatro categorias diarreiogênicas foram detectadas entre os isolados de E. coli obtidos de indivíduos infectados pelo HIV, com EIEC e EPEC representando os patótipos mais frequentes. Duas STEC stx1 e eae positivas foram identificadas, sendo este o primeiro relato de isolamento dessa categoria em indivíduos infectados pelo HIV no estado do Pará. A PCR multiplex mostrou-se como técnica eficiente, rápida e reprodutível para detecção dos isolados de DEC. Os dois sistemas de PCR multiplex apresentaram resultados 100% concordantes com os encontrados na PCR simples. CONCLUSÃO: Devido sua simplicidade, economia e eficiência torna-se possível a aplicação dos protocolos multiplex para agilizar o diagnóstico molecular das categorias de DEC, o que promoverá a elaboração de novos projetos de pesquisa e darão suporte as atividades de vigilância epidemiológica desenvolvidos pelos institutos de saúde pública]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[INTRODUCTION: Diarrheagenic Escherichia coli (DEC) are considered an important cause of diarrhea in developing countries. The correct identification of these microorganisms depends on their differentiation from non-pathogenic members of the intestinal microbiota. DEC can be classified into one of six categories according to their mechanism of pathogenicity. MATERIALS AND METHODS: Two multiplex PCR systems used to detect enteropathogenic (EPEC), enterotoxigenic (ETEC), enteroinvasive (EIEC) and Shiga Toxin-producing (STEC) E. coli were evaluated and described. RESULTS: Four categories of DEC were detected among isolates of E. coli obtained from individuals infected with HIV. EIEC and EPEC were among the most prevalent pathotypes. Furthermore, two STEC strains that were both stx1-and eae-positive were identified. This is the first report of this kind of isolation in individuals infected with HIV in the State of Pará. Multiplex PCR proved to be an efficient, fast and reproducible technique for detection of DEC isolates. Both multiplex PCR systems described here produced results 100% similar to those obtained from individual PCR reactions. CONCLUSION: Given their simplicity, cost and efficiency, it is possible to use these protocols to expedite the molecular diagnosis of the distinct categories of DEC. <body><![CDATA[<p><font size="2" face="Verdana">As <i><i>Escherichia coli</i></i> diarreiog&ecirc;nicas    (<i>diarrheagenic <i>E. coli</i></i> &#8211; DEC) representam importante causa de diarreia    end&ecirc;mica e epid&ecirc;mica no mundo<sup>29</sup>. Por&eacute;m provavelmente    a frequ&ecirc;ncia desses pat&oacute;genos &eacute; subestimada, devido sua    detec&ccedil;&atilde;o exigir m&eacute;todos diagn&oacute;sticos espec&iacute;ficos    que n&atilde;o s&atilde;o utilizados na pr&aacute;tica cl&iacute;nica.</font></p>     <p><font size="2" face="Verdana">A identifica&ccedil;&atilde;o de <i><i>E. coli</i></i>    diarreiog&ecirc;nica requer sua diferencia&ccedil;&atilde;o das esp&eacute;cies    n&atilde;o patog&ecirc;nicas da microbiota intestinal<sup>43</sup>. As <i><i>E. coli</i></i> que causam diarreia s&atilde;o classificadas em categorias patog&ecirc;nicas    de acordo com fatores de virul&ecirc;ncia espec&iacute;ficos, codificados por    cromossomos, plasm&iacute;dios e DNA de bacteri&oacute;fagos. Esses fatores    de virul&ecirc;ncia fornecem a cada categoria uma capacidade de causar s&iacute;ndrome    cl&iacute;nica com caracter&iacute;sticas epidemiol&oacute;gicas e patol&oacute;gicas    distintas<sup>38</sup>.</font></p>     <p><font size="2" face="Verdana">De acordo com seus mecanismos de patogenicidade    as DEC podem ser classificadas em: <i><i>E. coli</i></i> enteropatog&ecirc;nica (EPEC);    <i><i>E. coli</i></i> enterotoxig&ecirc;nica (ETEC); <i><i>E. coli</i></i> enteroinvasora    (EIEC); <i><i>E. coli</i></i> produtora da toxina de Shiga (STEC); <i><i>E. coli</i></i> enteroagregativa    (EAEC) e <i><i>E. coli</i></i> difusamente aderente (DAEC)<sup>29,42,48</sup>. Essas    categorias podem ser eventualmente identificadas atrav&eacute;s de ensaios sorot&iacute;picos,    fenot&iacute;picos e moleculares<sup>29</sup>. O primeiro requer a identifica&ccedil;&atilde;o    de ant&iacute;genos som&aacute;ticos (O) e flagelares (H). Na rotina laboratorial,    &eacute; realizada apenas a pesquisa dos sorogrupos pela determina&ccedil;&atilde;o    dos ant&iacute;genos som&aacute;ticos. Este, por&eacute;m, n&atilde;o &eacute;    suficiente para identificar uma amostra como diarreiog&ecirc;nica, devido n&atilde;o    estar correlacionado, em alguns casos, com a presen&ccedil;a de fatores de virul&ecirc;ncia<sup>24</sup>.    Portanto, a identifica&ccedil;&atilde;o deve ser direcionada para as caracter&iacute;sticas    que determinam a virul&ecirc;ncia desses microrganismos.</font></p>     <p><font size="2" face="Verdana">Os ensaios fenot&iacute;picos para a detec&ccedil;&atilde;o    de toxinas, padr&atilde;o de ader&ecirc;ncia ou invas&atilde;o podem identificar    amostras de <i><i>E. coli</i></i> diarreiog&ecirc;nicas, por&eacute;m requerem elevados    investimentos, t&eacute;cnicas especiais e aplica&ccedil;&atilde;o de v&aacute;rios    sistemas de detec&ccedil;&atilde;o (ex.: cultura de c&eacute;lulas, ensaios    de citotoxidade), al&eacute;m de consumo de longo tempo<sup>2,25</sup>. Com    a aplica&ccedil;&atilde;o da rea&ccedil;&atilde;o em cadeia da polimerase (<i>Polymerase    Chain Reaction</i> &#8211; PCR) &eacute; poss&iacute;vel detectar genes envolvidos    com a patogenicidade de diversos isolados bacterianos, permitindo simples identifica&ccedil;&atilde;o<sup>34</sup>.</font></p>     <p><font size="2" face="Verdana">Na investiga&ccedil;&atilde;o de pat&oacute;genos    a PCR &eacute; indicada para a amplifica&ccedil;&atilde;o de uma regi&atilde;o    espec&iacute;fica do DNA que permite a detec&ccedil;&atilde;o de determinado    locus de virul&ecirc;ncia. T&eacute;cnicas de PCR convencionais para a pesquisa    de <i><i>E. coli</i></i> diarreiog&ecirc;nicas tem sido relatadas, por&eacute;m, a    investiga&ccedil;&atilde;o dos isolados bacterianos requer um grande n&uacute;mero    de rea&ccedil;&otilde;es individuais para a detec&ccedil;&atilde;o de v&aacute;rios    fatores de virul&ecirc;ncia, o que a torna laboriosa<sup>20</sup>. Para reduzir    o n&uacute;mero de testes necess&aacute;rios para a identifica&ccedil;&atilde;o    de DEC, v&aacute;rios sistemas de PCR de detec&ccedil;&atilde;o m&uacute;ltipla    (multiplex) para a amplifica&ccedil;&atilde;o simult&acirc;nea de dois ou mais    <i>loci</i> em uma &uacute;nica rea&ccedil;&atilde;o t&ecirc;m sido desenvolvidos<sup>25,27,50</sup>.    Entretanto, usualmente, mais de uma PCR multiplex &eacute; requerida<sup>43</sup>. Para    simplificar e diminuir o tempo para o diagn&oacute;stico diferencial, foram    desenvolvidos neste estudo, sistemas de PCR multiplex aplicados para identifica&ccedil;&atilde;o    das seguintes categorias de DEC, cujos marcadores de virul&ecirc;ncia j&aacute;    est&atilde;o bem definidos: ETEC, EPEC, EIEC e STEC.</font></p>     <p>&nbsp;</p>     <p><font size="3" face="Verdana"><b>MATERIAIS E M&Eacute;TODOS</b></font></p>     <p><font size="2" face="Verdana"><b>ISOLADOS BACTERIANOS</b></font></p>     <p><font size="2" face="Verdana">Este estudo incluiu 720 <i><i>E. coli</i></i> estocadas    na bacterioteca da Se&ccedil;&atilde;o de Bacteriologia e Micologia do Instituto    Evandro Chagas (IEC), caracterizadas por m&eacute;todos fenot&iacute;picos tradicionais    como descrito previamente<sup>5</sup>. As <i><i>E. coli</i></i> foram isoladas de amostras    fecais de 181 indiv&iacute;duos maiores de 18 anos de idade infectados pelo    HIV, durante os anos de 2000 a 2002. O projeto que deu origem a este trabalho    foi aprovado pelo Comit&ecirc; de &Eacute;tica em Pesquisa com Seres Humanos    do IEC sob parecer n<sup>o</sup> 02/2000 em 26 de maio de 2000.</font></p>     <p><font size="2" face="Verdana"><b>PCR</b></font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana">Para a investiga&ccedil;&atilde;o dos fatores    de virul&ecirc;ncia, as amostras foram repicadas para tubos de ensaio com &aacute;gar    TSI (Difco), incubadas a 37<sup>o</sup> C por 24 h e posteriormente repicadas    para tubos com &aacute;gar TSA (Difco) para extra&ccedil;&atilde;o do DNA. Para    a extra&ccedil;&atilde;o do DNA bacteriano utilizou-se a t&eacute;cnica de termo-extra&ccedil;&atilde;o,    tendo o crescimento bacteriano sido transferido para microtubos com 400 &micro;L    de &aacute;gua purificada esterilizada e submetido a 100<sup>o</sup> C por 10    min. Ap&oacute;s a fervura, a suspens&atilde;o foi congelada a -20<sup>o</sup>    C. Depois dessa etapa, a suspens&atilde;o foi descongelada e centrifugada a    13.000 rpm (16845xg) em microcentr&iacute;fuga (mod. NT-805, Nova T&eacute;cnica).</font></p>     <p><font size="2" face="Verdana">As categorias diarreiog&ecirc;nicas EPEC, ETEC,    EIEC e STEC foram investigadas atrav&eacute;s de realiza&ccedil;&atilde;o de    PCR simples para sete diferentes marcadores moleculares, de acordo com condi&ccedil;&otilde;es    estabelecidas em estudos anteriores (<a href="#q1">Quadro 1</a>).</font></p>     <p><a name="q1"></a></p>     <p align="center"><img src="/img/revistas/rpas/v1n2/2a09q1.gif" border="0"></p>     <p>&nbsp;</p>     <p>&nbsp;</p>     <p><font size="2" face="Verdana">As <i><i>E. coli</i></i> diarreiog&ecirc;nicas foram    classificadas conforme a presen&ccedil;a de genes de fatores de virul&ecirc;ncia,    de acordo com crit&eacute;rios previamente publicados (<a href="#q2">Quadro    2</a>)<sup>7</sup>.</font></p>     <p><a name="q2"></a></p>     <p align="center"><img src="/img/revistas/rpas/v1n2/2a09q2.gif" border="0"></p>     <p>&nbsp;</p>     ]]></body>
<body><![CDATA[<p>&nbsp;</p>     <p><font size="2" face="Verdana">Cada rea&ccedil;&atilde;o apresentou volume final    de 50 &micro;l contendo 1 mM de deoxinucleot&iacute;deos (Invitrogen), 10 pmol    de cada oligonucleot&iacute;deo (Invitrogen), 1,5 mM de MgCl (Invitrogen), tamp&atilde;o    de Taq DNA polimerase 1&times; 2 (Invitrogen), 0,25 U de Taq DNA polimerase    (Invitrogen), e 3 &micro;l de DNA. As condi&ccedil;&otilde;es de amplifica&ccedil;&atilde;o    compreenderam desnatura&ccedil;&atilde;o inicial a 94&deg; C por 5 min, 35 ciclos    com 1,5 min a 94<sup>o</sup> C, 1,5 min a 50<sup>o</sup>/56<sup>o</sup> C e 1,5 min a 72<sup>o</sup> C e    extens&atilde;o final por 10 min a 72&deg; C. As rea&ccedil;&otilde;es foram    realizadas em termociclador (mod. CE 3832, Thermo Hyband). Os produtos de PCR    foram separados por eletroforese em gel de agarose D-1 LE GQT a 2% (BioAmerica)    contendo 1 &micro;g/mL de brometo de et&iacute;deo (Sigma) e visualizados em    transiluminador UVP (BioImaging Systems).</font></p>     <p><font size="2" face="Verdana">Para desenvolver os ensaios de PCR multiplex    foram testadas v&aacute;rias temperaturas de anelamento, concentra&ccedil;&otilde;es    de reagentes e incorpora&ccedil;&atilde;o progressiva dos oligonucleot&iacute;deos.    A sensibilidade e a especificidade da rea&ccedil;&atilde;o foram ensaiadas com    as cepas de refer&ecirc;ncias E2348/69 (EPEC), H10407 (ETEC LT-I/STa), EDL1284    (EIEC) e EDL933 (STEC). A cepa de <i>E. coli</i> K12 foi usada como controle negativo.    Os oligonucleot&iacute;deos utilizados est&atilde;o descritos no <a href="#q1">quadro    1</a>.</font></p>     <p>&nbsp;</p>     <p><font size="3" face="Verdana"><b>RESULTADOS</b></font></p>     <p><font size="2" face="Verdana">Para identifica&ccedil;&atilde;o simult&acirc;nea    e diferencia&ccedil;&atilde;o de quatro categorias diarreiog&ecirc;nicas de    <i>E. coli</i> foram desenvolvidos dois sistemas de PCR multiplex (Sistemas 1 e 2)    contendo, respectivamente, tr&ecirc;s e quatro pares de iniciadores (<a href="#q3">Quadro    3</a>).</font></p>     <p><a name="q3"></a></p>     <p align="center"><img src="/img/revistas/rpas/v1n2/2a09q3.gif" border="0"></p>     <p>&nbsp;</p>     <p>&nbsp;</p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana">Quando os pares de oligonucleot&iacute;deos foram    incorporados nos sistemas multiplex, uma interfer&ecirc;ncia foi observada.    Este foi o caso de LT-1/LT-2 que sob a concentra&ccedil;&atilde;o de 10 pmol/&micro;L    inibiu a amplifica&ccedil;&atilde;o de <i>eae</i>. Nesta situa&ccedil;&atilde;o, a    modifica&ccedil;&atilde;o da concentra&ccedil;&atilde;o de LT- 1/LT-2 para 5    pmol/&micro;L foi suficiente para favorecer a amplifica&ccedil;&atilde;o do    alvo esperado (<i>eae</i>).</font></p>     <p><font size="2" face="Verdana">Apenas os <i><i>loci</i></i> alvos foram detectados    nos controles positivos, sem que houvesse o aparecimento de amplifica&ccedil;&otilde;es    inespec&iacute;ficas. Como controle positivo dos sistemas de PCR multiplex foi    utilizada uma mistura contendo o DNA das cepas de refer&ecirc;ncia, observando-se    reprodutibilidade com os resultados obtidos na PCR simples. As amplifica&ccedil;&otilde;es    visualizadas nos sistemas 1 e 2 de PCR multiplex est&atilde;o demonstradas na    <a href="#f1">figura 1</a>.</font></p>     <p><a name="f1"></a></p>     <p align="center"><img src="/img/revistas/rpas/v1n2/2a09f1.gif" border="0"></p>     <p>&nbsp;</p>     <p>&nbsp;</p>     <p><font size="2" face="Verdana">A especificidade dos produtos obtidos nas rea&ccedil;&otilde;es    de PCR multiplex foi determinada pelo estudo das amostras de refer&ecirc;ncia.    Para valida&ccedil;&atilde;o dos sistemas de PCR multiplex desenvolvidos e demonstra&ccedil;&atilde;o    da utilidade diagn&oacute;stica, foram analisados os 78 isolados caracterizados    por PCR simples como DEC e o triplo de amostras (234 isolados) com resultados    de PCR negativos, compreendendo o total de 312 testes.</font></p>     <p><font size="2" face="Verdana">Dentre as 720 <i><i>E. coli</i></i> testadas    por PCR simples 78 (10,8%) apresentaram um ou mais genes caracter&iacute;sticos    de DEC, sendo identificadas em: EIEC (n = 34), EPEC (n = 29), ETEC (n = 13)    e STEC (n = 2). Das 29 EPEC encontradas 28 foram classificadas como EPEC at&iacute;picas,    pela aus&ecirc;ncia do gene bfpA (f&iacute;mbria BFP). As 13 ETEC foram caracterizadas    como LT-I (n = 2) e STa (n = 11). Um total de 34 isolados pertencia a categoria    EIEC. Duas STEC foram identificadas, sendo <i>stx1</i> e <i>eae</i> positivas.    Na an&aacute;lise comparativa dos dados obtidos foi encontrada concord&acirc;ncia    100% entre os resultados de PCR simples e multiplex.</font></p>     <p>&nbsp;</p>     <p><font size="3" face="Verdana"><b>DISCUSS&Atilde;O</b></font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana">Algumas DEC s&atilde;o geneticamente diferenciadas    das demais pela presen&ccedil;a de genes codificantes de fatores de virul&ecirc;ncia    espec&iacute;ficos que t&ecirc;m sido utilizados na identifica&ccedil;&atilde;o<sup>6,7,29,35,39</sup>.    Dentre esses, a detec&ccedil;&atilde;o simult&acirc;nea de m&uacute;ltiplos    genes de virul&ecirc;ncia atrav&eacute;s de ensaios de PCR multiplex tem sido    executada<sup>4,36,41</sup>. Para reduzir o n&uacute;mero de PCR realizadas,    n&oacute;s desenvolvemos dois sistemas de PCR multiplex para detec&ccedil;&atilde;o    de sete genes de fatores de virul&ecirc;ncia, caracter&iacute;sticos de quatro    pat&oacute;tipos: EPEC, ETEC, EIEC e STEC. Embora sejam conhecidas seis linhagens    de <i><i>E. coli</i></i> diarreiog&ecirc;nicas, somente aquelas quatro apresentam    marcadores de virul&ecirc;ncia bem definidos<sup>29</sup>.</font></p>     <p><font size="2" face="Verdana">Alguns estudos t&ecirc;m sugerido alvos moleculares    para caracteriza&ccedil;&atilde;o de EAEC, como os genes <i>setBA</i>, <i>aaiA</i>,    <i>astA</i> e <i>aat</i>. Entretanto, observou-se que alguns destes marcadores    n&atilde;o s&atilde;o exclusivos dessa categoria<sup>17,19</sup>. Os genes <i>afa</i>,    <i>dra</i> e <i>daa</i>, codificadores de uma fam&iacute;lia de adesinas, t&ecirc;m    sido utilizados para caracteriza&ccedil;&atilde;o de DAEC, por&eacute;m os mesmos    podem estar presentes em <i><i>E. coli</i></i> n&atilde;o patog&ecirc;nicas<sup>22</sup>.    Portanto, devido a essas peculiaridades, recomenda-se a inclus&atilde;o de ensaios    de ader&ecirc;ncias em cultura de c&eacute;lulas HEp-2 ou HeLa como m&eacute;todo    padr&atilde;o-ouro para defini&ccedil;&atilde;o das categorias EAEC e DAEC<sup>29</sup>.</font></p>     <p><font size="2" face="Verdana">S&atilde;o crescentes as descri&ccedil;&otilde;es    de sistemas de PCR para identifica&ccedil;&atilde;o simult&acirc;nea de m&uacute;ltiplos    <i><i>loci</i></i>. Pass et al<sup>33</sup> realizaram PCR multiplex para detec&ccedil;&atilde;o    de 11 diferentes fatores de virul&ecirc;ncia, entretanto utilizaram quatro sistemas,    o que representa pouca praticidade. Nguyen et al<sup>31</sup> apresentaram um    ensaio de PCR multiplex para detec&ccedil;&atilde;o de oito genes caracter&iacute;sticos    de EPEC, ETEC, EIEC, STEC e EAEC, por&eacute;m com baixa resolutividade entre    ETEC-LT e EIEC (diferen&ccedil;as de 2 pb entre os fragmentos caracter&iacute;sticos    dessas categorias). M&uuml;ller et al<sup>28</sup> tamb&eacute;m desenvolveram    um sistema de PCR multiplex para identifica&ccedil;&atilde;o de DEC, no entanto    aplicado apenas &agrave;s categorias EPEC e EIEC.</font></p>     <p><font size="2" face="Verdana">Outros estudos relataram o uso de PCR multiplex    para identifica&ccedil;&atilde;o de v&aacute;rios pat&oacute;tipos<sup>6,20,30,43,48</sup>.    Entretanto, apresentam como limita&ccedil;&atilde;o a inabilidade de diferenciar    todas as categorias de DEC, principalmente em infec&ccedil;&otilde;es mistas    de por EPEC at&iacute;picas e STEC, tornando necess&aacute;ria a realiza&ccedil;&atilde;o    de outras PCR para verifica&ccedil;&atilde;o dos resultados.</font></p>     <p><font size="2" face="Verdana">Neste estudo foram desenvolvidos dois sistemas    de PCR multiplex de r&aacute;pida otimiza&ccedil;&atilde;o, simples execu&ccedil;&atilde;o,    f&aacute;cil implanta&ccedil;&atilde;o na rotina de laborat&oacute;rios de refer&ecirc;ncia,    baratos &#8211; quando comparados aos sistemas de PCR que utilizam marcadores    fluorescentes &#8211; e que dispensam a realiza&ccedil;&atilde;o de PCR adicionais    para esclarecimento dos resultados, como por exemplo, na presen&ccedil;a de    pat&oacute;tipos que compartilhem marcadores de virul&ecirc;ncia. O m&eacute;todo    permitiu a identifica&ccedil;&atilde;o de quatro categorias diarreiog&ecirc;nicas,    representando 10,8% dos isolados, com predom&iacute;nio de EIEC seguido de EPEC,    com maior preval&ecirc;ncia da subcategoria at&iacute;pica.</font></p>     <p><font size="2" face="Verdana">As EIEC s&atilde;o capazes de invadir a mucosa    intestinal causando ocasionalmente quadro de disenteria. Estudos relatam a incid&ecirc;ncia    muito baixa para essa categoria, sendo a maior ocorr&ecirc;ncia relacionada    a surtos e entre crian&ccedil;as de pa&iacute;ses em desenvolvimento<sup>13,14,15,23,26</sup>.    Investiga&ccedil;&otilde;es realizadas no Brasil sobre enteropat&oacute;genos    envolvidos com diarreia infantil demonstraram baixa frequ&ecirc;ncia    de EIEC<sup>11,37</sup>, contrastando com os achados deste estudo. Entretanto,    tal diferen&ccedil;a pode estar associada &agrave; faixa et&aacute;ria e popula&ccedil;&atilde;o    pesquisada, sendo que naqueles estudos foram investigadas crian&ccedil;as e,    no presente trabalho, se avaliou isolados provenientes de indiv&iacute;duos    adultos infectados pelo HIV. Em estudo com indiv&iacute;duos infectados pelo    HIV no Senegal, o pat&oacute;tipo EIEC representou a segunda categoria de <i><i>E. coli</i></i> mais prevalente<sup>12</sup>, semelhante ao encontrado neste trabalho.</font></p>     <p><font size="2" face="Verdana">Dos 29 isolados de EPEC, 28 foram classificados    como EPEC at&iacute;picas. A divis&atilde;o de amostras de EPEC em t&iacute;picas    e at&iacute;picas tem importante implica&ccedil;&atilde;o cl&iacute;nico-epidemiol&oacute;gica,    antes n&atilde;o valorizada. Por muitos anos, alguns sorotipos de EPEC t&iacute;picas    associados &agrave; diarreia infantil foram isolados com frequ&ecirc;ncia,    mas tem sido observada a redu&ccedil;&atilde;o desta subcategoria e um aumento    relativo do isolamento de EPEC at&iacute;picas em muitos pa&iacute;ses<sup>1,18,21,40,45,46,49</sup>.    Alguns trabalhos sugerem que as EPEC at&iacute;picas sejam t&atilde;o patog&ecirc;nicas    quanto &agrave;s t&iacute;picas, embora tenha sido relatado que a diarreia    provocada por essa subcategoria seja mais branda<sup>51</sup>. H&aacute; registros    tamb&eacute;m de que as EPEC at&iacute;picas estejam mais significativamente    associadas &agrave; diarreia end&ecirc;mica ou como causa de surtos<sup>9,10,51</sup>.</font></p>     <p><font size="2" face="Verdana">Duas STEC <i>stx1</i> e <i><i>eae</i></i> positivas    foram identificadas. As infec&ccedil;&otilde;es devido a STEC s&atilde;o relatadas    em casos espor&aacute;dicos no Brasil<sup>3,6,11,16</sup>, sendo este o primeiro relato    de isolamento dessa categoria em indiv&iacute;duos infectados pelo HIV no Estado    do Par&aacute;.</font></p>     <p>&nbsp;</p>     <p><font size="3" face="Verdana"><b>CONCLUS&Atilde;O</b></font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana">Os resultados deste estudo demonstraram a diversidade    de pat&oacute;tipos de <i>E. coli</i> isolados de indiv&iacute;duos infectados pelo    HIV, com predom&iacute;nio de EIEC e EPEC at&iacute;picas.
