MÉTODOS: Foram calculados e avaliados os indicadores epidemiológicos do manual de leishmaniose tegumentar americana do Ministério da Saúde. Optou-se por uma série histórica de dez anos, com dados coletados no Departamento de Vigilância Epidemiológica da Secretária Estadual de Saúde de Alagoas. RESULTADOS: Constatou-se a notificação de 1.338 novos casos autóctones, dos quais 1.097 pacientes tinham idade igual ou maior que 10 anos. Quanto ao sexo, 525 pacientes eram do sexo feminino e 811 do sexo masculino. A forma cutânea foi diagnosticada em 969 pacientes, com pico em 2007, quando foram registrados 98,3% dos casos. Realizaram-se 625 intradermorreações de Montenegro, obtendo, no ano de 2008, um valor máximo de 99,1%. CONCLUSÃO: Observou-se em Alagoas um coeficiente de detecção de nível médio, com predomínio de homens maiores de 10 anos de idade e com a forma cutânea, apresentando uma tendência de diminuição no número de casos e aumento da proporção no sexo masculino na faixa de idade maior ou igual a 10 anos e nas lesões cutâneas. O conhecimento desse panorama auxilia as tomadas de decisões em saúde pública no Estado.]]></p></abstract>

It is one of the parasitic diseases that affect skin and mucosae, and is closely related to environmental, behavioral and demographic changes. METHODS: The epidemiological indicators of the manual for surveillance of American integumentary leishmaniasis were calculated and evaluated on a ten-year historical series and their data were collected by the Department of Epidemiological Surveillance of the Alagoas State Secretary of Health. RESULTS: A total of 1,338 new indigenous cases were notified: 1,097 patients were 10 years old or older; 525 patients were female and 811 male. Cutaneous leishmaniasis was diagnosed in 969 patients, with a peak in 2007, when 98.3% of the cases were reported; 625 Montenegro skin tests were performed, with a peak of 99.1% in 2008. CONCLUSIONS: In Alagoas, a medium level detection rate was observed, with a prevalence of the cutaneous type in male patients over 10 years old, and a decreasing trend in the case numbers and an increase in the proportion of males in the age group of &#8805; 10 years old and in the cutaneous lesions. Knowing this panorama supports decision making processes in the public health field within the state.]]></p></abstract>

MÉTODOS: Fueron calculados y evaluados los indicadores epidemiológicos del manual de leishmaniasis tegumentar americana del Ministerio de Salud. Se optó por una serie histórica de diez años, con datos recolectados en el Departamento de Vigilancia Epidemiológica de la Secretaría Estadual de Salud de Alagoas. RESULTADOS: Fue constatada la notificación de 1.338 nuevos casos autóctonos, de los cuales 1.097 pacientes tenían edad igual o superior a 10 años. Con relación a sexo, 525 pacientes eran del sexo femenino y 811 del sexo masculino. La forma cutánea fue diagnosticada en 969 pacientes, con un pico en 2007, cuando se registró un 98,3% de los casos. Fueron realizadas 625 intradermo reacciones de Montenegro, obteniendo, el año de 2008, un valor máximo de 99,1%. CONCLUSIÓN: Se observó en Alagoas un coeficiente de detección de nivel medio, con predominio de hombres mayores de 10 años de edad y de la forma cutánea, presentando una tendencia de disminución en el número de casos y aumento de la proporción del sexo masculino en la franja de edad mayor o igual a 10 años y en las lesiones cutáneas. O protozo&aacute;rio respons&aacute;vel pelo quadro  cl&iacute;nico pertence ao g&ecirc;nero <i>Leishmania, </i>tratando-se de um parasito  intracelular obrigat&oacute;rio de macr&oacute;fagos<sup>1</sup>. Trata-se de uma zoonose com ampla  distribui&ccedil;&atilde;o geogr&aacute;fica e com manifesta&ccedil;&otilde;es cl&iacute;nicas que podem evoluir para  les&otilde;es que mutilam o paciente. Al&eacute;m deste aspecto, a LTA est&aacute; intimamente  relacionada &agrave;s altera&ccedil;&otilde;es ambientais, comportamentais humanas e demogr&aacute;ficas,  estando em evid&ecirc;ncia como um grave problema de sa&uacute;de p&uacute;blica no  Brasil<sup>2,3</sup>.</font></p>     <p><font size="2" face="Verdana">&Eacute; uma  doen&ccedil;a que vem sendo notificada em todos os Estados brasileiros<sup>4</sup>,  atingindo indiv&iacute;duos de diferentes faixas et&aacute;rias e em ambos os sexos<sup>5,6,7</sup>.  O Brasil, juntamente com outros cinco pa&iacute;ses, det&eacute;m 90% dos casos de  leishmaniose cut&acirc;nea do mundo<sup>8,9</sup>. A Regi&atilde;o Nordeste responde por 39% dos casos notificados  de LTA do Brasil<sup>10</sup>.</font></p>     <p><font size="2" face="Verdana">A  prov&aacute;vel esp&eacute;cie respons&aacute;vel pela transmiss&atilde;o de LTA em Alagoas &eacute; a <i>L. (V.)  braziliensis, </i>a qual est&aacute; presente em todos os Estados do Pa&iacute;s<sup>11</sup>  e &eacute; a de mais vasta distribui&ccedil;&atilde;o geogr&aacute;fica entre os agentes causadores de LTA  no mundo<sup>12</sup>.</font></p>     <p><font size="2" face="Verdana">O  estudo dos indicadores epidemiol&oacute;gicos e operacionais da LTA em Alagoas numa  s&eacute;rie hist&oacute;rica de dez anos permite avaliar a din&acirc;mica da endemia e planejar  medidas de controle, visando diminuir seu impacto sobre a popula&ccedil;&atilde;o, auxiliando  na tomada de decis&atilde;o em sa&uacute;de p&uacute;blica<sup>9</sup> nas esferas da vigil&acirc;ncia, da  assist&ecirc;ncia e controle. Os resultados obtidos neste estudo poder&atilde;o ser  expandidos para os Estados vizinhos, onde as condi&ccedil;&otilde;es epidemiol&oacute;gicas s&atilde;o  semelhantes &agrave;s de Alagoas.</font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana">Foram  analisados os dados referentes aos coeficientes de detec&ccedil;&atilde;o, das faixas  et&aacute;rias, do sexo, das manifesta&ccedil;&otilde;es (formas) cl&iacute;nicas, da densidade, dos  m&eacute;todos de aux&iacute;lio diagn&oacute;stico e da propor&ccedil;&atilde;o de cura cl&iacute;nica<sup>11</sup>.</font></p>     <p>&nbsp;</p>     <p><font size="3" face="Verdana"><b>M&Eacute;TODOS</b></font></p>     <p><font size="2" face="Verdana">Trata-se de um estudo que calcula e avalia os indicadores epidemiol&oacute;gicos contidos  no manual de leishmaniose tegumentar americana do Minist&eacute;rio da Sa&uacute;de (MS). Optou-se por um per&iacute;odo de dez anos (janeiro de 1999  - dezembro  de 2008) no Estado de Alagoas, abrangendo todas as notifica&ccedil;&otilde;es de  novos casos realizadas nessa d&eacute;cada, em todos os 102 munic&iacute;pios do Estado.</font></p>     <p><font size="2" face="Verdana">Os  dados foram coletados  no Departamento de Vigil&acirc;ncia Epidemiol&oacute;gica da  Secret&aacute;ria Estadual de Sa&uacute;de de Alagoas, pelo Sistema de Informa&ccedil;&atilde;o de Agravos  de Notifica&ccedil;&atilde;o (SINAN). A partir dos dados obtidos foram calculados os  indicadores.</font></p>     <p><font size="2" face="Verdana">O  Manual apresenta 11 pontos a serem analisados. S&atilde;o eles: n&uacute;mero de novos casos  aut&oacute;ctones notificados na unidade federada no ano; coeficiente geral de  detec&ccedil;&atilde;o de casos na unidade federada por 100 mil habitantes; propor&ccedil;&atilde;o anual  de casos na faixa et&aacute;ria menor de 10 anos entre o total de casos diagnosticados  no ano; propor&ccedil;&atilde;o anual de casos na faixa et&aacute;ria de 10 anos ou mais entre o  total de casos diagnosticados no ano; propor&ccedil;&atilde;o anual de casos no sexo feminino  entre o total de casos diagnosticados no ano; propor&ccedil;&atilde;o anual de casos no sexo  masculino entre o total de casos diagnosticados no ano; propor&ccedil;&atilde;o de casos da  forma mucosa entre o total de casos diagnosticados; propor&ccedil;&atilde;o de casos da forma  cut&acirc;nea entre o total de casos diagnosticados no ano; densidade de casos;  propor&ccedil;&atilde;o de casos novos submetidos a m&eacute;todos auxiliares de diagn&oacute;stico, entre  o total de casos diagnosticados no ano; propor&ccedil;&atilde;o de casos que evolu&iacute;ram para  cura cl&iacute;nica entre o total de casos registrados do per&iacute;odo<sup>11</sup>.</font></p>     <p><font size="2" face="Verdana">Para o  coeficiente geral de detec&ccedil;&atilde;o foram utilizadas as estimativas populacionais do  banco de dados do Sistema &Uacute;nico de Sa&uacute;de (DATASUS) de 1999 a 2008.</font></p>     <p><font size="2" face="Verdana">Foram  realizadas distribui&ccedil;&otilde;es de frequ&ecirc;ncias, regress&otilde;es lineares, c&aacute;lculo das  m&eacute;dias, desvios padr&otilde;es e percentuais dos dados, para a an&aacute;lise estat&iacute;stica do  estudo.</font></p>     <p>&nbsp;</p>     <p><font size="3" face="Verdana"><b>RESULTADOS</b></font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana">No per&iacute;odo de 1999 a 2008, em Alagoas,  foram notificados 1.338 casos  de LTA, apresentando um coeficiente de detec&ccedil;&atilde;o que variou de 1,5 a 11,0 por 100 mil habitantes,  segundo dados da Secretaria Estadual de Sa&uacute;de de Alagoas. A densidade de casos de  LTA, o coeficiente geral de detec&ccedil;&atilde;o de casos e o n&uacute;mero de novos casos  aut&oacute;ctones s&atilde;o observados na <a href="#t1">tabela 1</a>.</font> <font size="2" face="Verdana">Inicia-se em ascens&atilde;o, atingindo o valor m&aacute;ximo no ano  de 2000, para todas as vari&aacute;veis. Logo ap&oacute;s, seguem-se per&iacute;odos alternando  eleva&ccedil;&atilde;o e decr&eacute;scimo at&eacute; atingirem o valor m&iacute;nimo para as tr&ecirc;s vari&aacute;veis no  ano de 2006,  seguindo-se leve ascens&atilde;o e, de novo, decr&eacute;scimo nas tr&ecirc;s  vari&aacute;veis.</font></p>     <p><a name="t1" id="t1"></a></p>     <p>&nbsp;</p>     <p align="center"><img src="/img/revistas/rpas/v1n3/3a13t1.gif" border="0"></p>     <p>&nbsp;</p>     <p><font size="2" face="Verdana">Na  <a href="#f1">figura 1</a> s&atilde;o abordadas as vari&aacute;veis de propor&ccedil;&atilde;o anual de LTA em pessoas com  idade menor que 10 anos e em pessoas com idade igual ou maior que 10 anos; e  propor&ccedil;&atilde;o anual de LTA no sexo feminino e no sexo masculino. Observa-se que, num total de 241 pacientes menores de  10 anos de idade, houve varia&ccedil;&otilde;es para mais ou para menos nas ocorr&ecirc;ncias, mas  sempre situadas entre 21,73%  no ano de 1999 e 11,56% no ano de 2002, valores m&aacute;ximo e  m&iacute;nino, respectivamente (<a href="#f1">Figura 1</a>). Tamb&eacute;m no total de 1.097 pacientes com  idade igual ou maior que 10 anos, ocorrem varia&ccedil;&otilde;es para mais ou para menos nos  valores compreendidos entre 88,42% no ano de 2002 &mdash; valor m&aacute;ximo &mdash; e 78,26% no ano de 1999 &mdash; valor m&iacute;nimo. Nas  propor&ccedil;&otilde;es referentes ao sexo trabalhou-se com 525 pacientes do sexo feminino e  811 do sexo masculino. Em ambos os sexos houve altern&acirc;ncia de per&iacute;odos de  aumento na propor&ccedil;&atilde;o com per&iacute;odos de decl&iacute;nio. Para o sexo feminino, os valores  m&aacute;ximo e m&iacute;nimo foram, respectivamente, 47,82%, no ano de 1999, e 28,44% no ano  de 2008; e para o sexo masculino, os valores m&aacute;ximo e m&iacute;nimo foram 71,55%, em 2008, e 51,73%, em 1999.</font></p>     <p><a name="f1" id="f1"></a></p>     <p>&nbsp;</p>     <p align="center"><img src="/img/revistas/rpas/v1n3/3a13f1.gif" border="0"></p>     <p>&nbsp;</p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana">A  <a href="#f2">figura 2</a> retrata a propor&ccedil;&atilde;o de casos novos diagnosticados de LTA na forma  cut&acirc;nea e na forma mucosa, e a propor&ccedil;&atilde;o de casos que evolu&iacute;ram para a cura  cl&iacute;nica. Todas apresentaram varia&ccedil;&otilde;es de eleva&ccedil;&atilde;o e decr&eacute;scimo, como retratado  na figura. A forma mucosa foi diagnosticada em 70 pacientes com valor m&aacute;ximo no  ano de 2006, correspondendo  a 17% dos casos, e valor  m&iacute;nimo em 2007, com 1,62% dos casos. Na forma  cut&acirc;nea &mdash; foram 969 pacientes &mdash; o pico m&aacute;ximo ocorreu  no ano de 2007, com 98,3% dos casos e o pico  m&iacute;nimo em 1999, com 51,7% dos casos. A alta por  cura cl&iacute;nica apresentou pico m&aacute;ximo em 2008 e m&iacute;nimo em 2001.</font></p>     <p><a name="f2" id="f2"></a></p>     <p>&nbsp;</p>     <p align="center"><img src="/img/revistas/rpas/v1n3/3a13f2.gif" border="0"></p>     <p>&nbsp;</p>     <p><font size="2" face="Verdana">Na  <a href="#f3">figura 3</a> s&atilde;o  mostradas as propor&ccedil;&otilde;es de casos novos submetidos a m&eacute;todos auxiliares de  diagn&oacute;stico entre o total de diagnosticados no ano. Esses m&eacute;todos s&atilde;o: a  intradermorrea&ccedil;&atilde;o de Montenegro (IDRM) e os exames histopatol&oacute;gico e parasitol&oacute;gico  direto. A IDRM  foi realizada em 625 pacientes,  e apresentou tend&ecirc;ncia geral de ascens&atilde;o, como pode ser visto na figura onde se  registram apenas dois pontos de varia&ccedil;&atilde;o para menor, os anos de 2000 e 2004, o primeiro sendo o  pico m&iacute;nimo, correspondendo a 16,4% dos casos. O pico m&aacute;ximo foi observado no ano de 2008,  com uma cobertura de 99,1% dos casos. O exame histopatol&oacute;gico foi realizado em  225 casos, sendo seu valor m&aacute;ximo, 56,84%, no ano de 2002, e seu valor m&iacute;nimo  no ano de 2000, 7,71%, com os demais valores situados nesta faixa. J&aacute; o exame parasitol&oacute;gico  direto tamb&eacute;m  apresenta oscila&ccedil;&otilde;es, tendo o valor m&aacute;ximo, 44,55% ocorrido no ano de 2006, e o valor m&iacute;nimo, 7,4%, no ano de 2000, com um total de 205 realiza&ccedil;&otilde;es.</font></p>     <p><a name="f3" id="f3"></a></p>     <p>&nbsp;</p>     <p align="center"><img src="/img/revistas/rpas/v1n3/3a13f3.gif" border="0"></p>     <p>&nbsp;</p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana">Por meio das regress&otilde;es lineares (<a href="#f4">Figura 4</a>) &eacute; poss&iacute;vel observar  que o n&uacute;mero de casos notificados apresenta uma tend&ecirc;ncia &agrave; diminui&ccedil;&atilde;o, contudo  ocorre uma predomin&acirc;ncia crescente no sexo masculino, nos pacientes com idade  maior ou igual a 10 anos e na forma cut&acirc;nea da doen&ccedil;a em rela&ccedil;&atilde;o &agrave; forma  mucosa.</font></p>     <p><a name="f4" id="f4"></a></p>     <p>&nbsp;</p>     <p align="center"><img src="/img/revistas/rpas/v1n3/3a13f4.gif" border="0"></p>     <p>&nbsp;</p>     <p><font size="2" face="Verdana"><b>&nbsp;</b></font></p>     <p><font size="3" face="Verdana"><b>DISCUSS&Atilde;O</b></font></p>     <p><font size="2" face="Verdana">No  Estado de Alagoas, o coeficiente de detec&ccedil;&atilde;o de LTA mostrou-se, ao longo da d&eacute;cada  analisada, predominantemente no segundo quartil. Esse par&acirc;metro de  classifica&ccedil;&atilde;o enquadra Alagoas em n&iacute;vel m&eacute;dio no que se refere a esse indicador<sup>11</sup>.  Ao longo de 10 anos o  coeficiente m&eacute;dio de detec&ccedil;&atilde;o de LTA firmou-se em 4,6 casos por 100 mil habitantes.  Comparando o coeficiente de detec&ccedil;&atilde;o deste trabalho, encontrou-se uma diverg&ecirc;ncia com  os valores do SINAN, divulgados pelo MS, onde a m&eacute;dia foi de 3,6/100 mil habitantes. Em  rela&ccedil;&atilde;o &agrave; Regi&atilde;o Nordeste (17,2), Alagoas situa-se na quinta posi&ccedil;&atilde;o, estando  com os maiores coeficientes os Estados do Maranh&atilde;o (58,2), Cear&aacute; (22,1), Bahia (17,8) e Pernambuco (7,4).  Valores inferiores foram encontrados nos Estados do Piau&iacute; (4,5), Para&iacute;ba (2,0)  e Rio Grande do Norte (0,3). No Brasil, a m&eacute;dia do coeficiente de detec&ccedil;&atilde;o do  per&iacute;odo estudado foi 15,7.</font></p>     <p><font size="2" face="Verdana">Esse  valor caracteriza o Estado como &aacute;rea end&ecirc;mica para LTA, por&eacute;m mostrando uma  tend&ecirc;ncia de diminui&ccedil;&atilde;o do n&uacute;mero de casos<sup>13</sup>. Por se tratar de uma  doen&ccedil;a multifatorial, v&aacute;rios aspectos, isoladamente ou associados, tais como:  redu&ccedil;&atilde;o da popula&ccedil;&atilde;o rural, consolida&ccedil;&atilde;o das &aacute;reas desmatadas, o uso de  inseticidas em algumas &aacute;reas, dentre outros, podem ter contribu&iacute;do com essa  redu&ccedil;&atilde;o. Outros fatores (ambientais e ecol&oacute;gicos) podem ter influenciado no  decr&eacute;scimo do n&uacute;mero de casos, uma vez que a varia&ccedil;&atilde;o anual tem ocorrido em outros Estados.</font></p>     <p><font size="2" face="Verdana">A  preval&ecirc;ncia de casos no sexo masculino obteve m&eacute;dia de 61,3% em uma d&eacute;cada,  por&eacute;m inferior &agrave; m&eacute;dia nacional, que se situa acima dos 70%. Considerando que o  Estado de Alagoas sofreu forte impacto de desmatamento, em decorr&ecirc;ncia da  expans&atilde;o da cultura da cana-de-a&ccedil;&uacute;car, o componente florestal pode estar  contribuindo menos na preval&ecirc;ncia masculina da doen&ccedil;a.</font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana">No que  se refere &agrave; faixa et&aacute;ria mais elevada ocorreram oscila&ccedil;&otilde;es entre 78,3% no ano  de 1999 at&eacute; 88,4% em 2002, o que corrobora a informa&ccedil;&atilde;o que homens adolescentes  e adultos s&atilde;o os mais acometidos, geralmente trabalhadores de &aacute;reas rurais e  maiores de 10 anos de idade, em idade reprodutiva e laborativa<sup>14,15</sup>,  relacionados principalmente com planta&ccedil;&otilde;es de subsist&ecirc;ncia, principalmente de  bananeira, em &aacute;reas de desmatamento antigo<sup>16,17</sup>.</font></p>     <p><font size="2" face="Verdana">Observa-se  que h&aacute; uma tend&ecirc;ncia na eleva&ccedil;&atilde;o da preval&ecirc;ncia em idade maior ou igual a 10  anos e no sexo masculino, uma vez que homens adolescentes e adultos desenvolvem  atividades laborais mais afastados do domic&iacute;lio do que as crian&ccedil;as e as  mulheres.</font></p>     <p><font size="2" face="Verdana">A  ocorr&ecirc;ncia de casos em mulheres, crian&ccedil;as, adolescentes e idosos sugere  evid&ecirc;ncia de transmiss&atilde;o peridomiciliar<sup>18,19</sup>, devido &agrave; localiza&ccedil;&atilde;o  das resid&ecirc;ncias pr&oacute;ximas dos bananeirais ou matas remanescentes, como tamb&eacute;m  pela possibilidade de adapta&ccedil;&atilde;o de vetores<sup>16,17,12</sup>. Entretanto,  parece n&atilde;o haver varia&ccedil;&atilde;o durante o per&iacute;odo estudado. Apesar de Ampuero et al<sup>20</sup>,  terem referido a possibilidade da presen&ccedil;a de um adulto infectado como  hospedeiro intradomiciliar da <i>Leishmania, </i>historicamente n&atilde;o tem sido  essa a forma de manuten&ccedil;&atilde;o do ciclo de transmiss&atilde;o de LTA no Pa&iacute;s. De uma  maneira geral, a transmiss&atilde;o &eacute; vetorial e em &aacute;reas de coloniza&ccedil;&atilde;o antiga, em  que os animais dom&eacute;sticos podem servir como fonte de atra&ccedil;&atilde;o aos vetores. Mas  mesmo nestas &aacute;reas n&atilde;o &eacute; f&aacute;cil mostrar que existe domicilia&ccedil;&atilde;o dos vetores da  LTA, muito menos o homem servindo como disseminador da doen&ccedil;a. Em geral os  homens s&atilde;o alcan&ccedil;ados pelo raio de dispers&atilde;o dos vetores, que v&ecirc;m atra&iacute;dos pela  luz, presen&ccedil;a de animais dom&eacute;sticos e eventualmente do pr&oacute;prio homem.</font></p>     <p><font size="2" face="Verdana">O  aparecimento de les&atilde;o em mucosas mant&eacute;m-se sempre abaixo dos 10%, com uma  m&eacute;dia, em 10 anos, de 6,8% dos casos, exceto no ano de 2006, quando atingiu a marca  de 17%. Esse dado coincide  com outros estudos, quanto &agrave; frequ&ecirc;ncia de les&otilde;es em mucosa de pacientes com LTA<sup>10,7</sup>. A preval&ecirc;ncia da forma mucosa detectada em Alagoas &eacute; semelhante &agrave; da m&eacute;dia  nacional nos anos de 2001 a 2006, que foi de 6,3%. No ano de 2006 houve um acr&eacute;scimo na  detec&ccedil;&atilde;o de pacientes com les&atilde;o mucosa (17%); isso pode ter ocorrido em  decorr&ecirc;ncia de uma maior atividade do programa estadual de endemias nesse ano,  tendo como consequ&ecirc;ncia o crescimento do conhecimento sobre a doen&ccedil;a, como  tamb&eacute;m do diagn&oacute;stico e tratamento precoces<sup>21</sup>. As les&otilde;es mucosas  aparecem geralmente ap&oacute;s um per&iacute;odo de cinco anos do surgimento da les&atilde;o  prim&aacute;ria<sup>1</sup> e s&atilde;o favorecidas tamb&eacute;m em pessoas desnutridas<sup>7</sup>.</font></p>     <p><font size="2" face="Verdana">O  acometimento de mucosas est&aacute; relacionado provavelmente a infec&ccedil;&atilde;o pela <i>L.  (V.) braziliensis</i><sup>22,15,23</sup>, o que refor&ccedil;a a possibilidade desta  esp&eacute;cie ser a principal respons&aacute;vel pela LTA em Alagoas. Essa forma  est&aacute; associada &agrave; persist&ecirc;ncia do parasito nos tecidos com posterior migra&ccedil;&atilde;o  pela corrente sangu&iacute;nea como tamb&eacute;m pela via linf&aacute;tica<sup>24</sup>. Apesar de  n&atilde;o existirem estudos em Alagoas identificando a esp&eacute;cie respons&aacute;vel pela LTA,  segundo Reis et al<sup>3</sup>, no Estado de Pernambuco, que faz parte do mesmo  circuito de transmiss&atilde;o de Alagoas, a <i>L. (V.) braziliensis </i>foi o &uacute;nico  parasito encontrado.</font></p>     <p><font size="2" face="Verdana">Com a  forma cut&acirc;nea, a express&atilde;o da doen&ccedil;a d&aacute;-se tanto por meio de les&otilde;es, &uacute;nica  ou m&uacute;ltiplas, e representa a forma mais frequente da doen&ccedil;a<sup>8,25</sup>. No  presente estudo notou-se  que desde 2002 - exceto em 2006 - a notifica&ccedil;&atilde;o da  forma cut&acirc;nea esteve acima de 90%<sup>7,23</sup>. Logo, a import&acirc;ncia do  tratamento da les&atilde;o cut&acirc;nea tem como objetivo: impedir a invas&atilde;o pelo parasito  da mucosa, acelerar o processo de cicatriza&ccedil;&atilde;o e evitar a forma&ccedil;&atilde;o de  cicatrizes desfigurantes<sup>26</sup>.</font></p>     <p><font size="2" face="Verdana">Percebe-se que no per&iacute;odo de dez anos as altas  cl&iacute;nicas aumentaram, principalmente no per&iacute;odo de 2003 a 2008, sendo  observado, no &uacute;ltimo ano, um &iacute;ndice de 91,7% de pacientes curados e que  receberam alta m&eacute;dica. O crit&eacute;rio de cura do paciente &eacute; cl&iacute;nico, sendo  recomendado monitoramento mensal nos tr&ecirc;s primeiros meses e, com a cura cl&iacute;nica  alcan&ccedil;ada, o acompanhamento deve seguir por at&eacute; um ano ap&oacute;s o final do  tratamento<sup>27,11</sup>. Esse crescimento est&aacute; associado ao acompanhamento  cl&iacute;nico, ao uso da medica&ccedil;&atilde;o e rapidez do in&iacute;cio do tratamento<sup>21</sup>.  Isso leva o paciente a aderir ao tratamento com maior facilidade, apesar dos  efeitos colaterais apresentados pela medica&ccedil;&atilde;o em alguns pacientes, como tamb&eacute;m  reduz a velocidade de ulcera&ccedil;&atilde;o da les&atilde;o e previne a dissemina&ccedil;&atilde;o do parasito<sup>9</sup>.  Outro aspecto relacionado ao aumento do percentual de encerramento dos casos  por cura &eacute; o reflexo de uma melhora no seguimento dos pacientes,  consequentemente, na informa&ccedil;&atilde;o contida no SINAN, j&aacute; que o n&uacute;mero de &oacute;bitos (9)  e de abandono (17) foi muito baixo.</font></p>     <p><font size="2" face="Verdana">O diagn&oacute;stico da LTA &eacute; eminentemente cl&iacute;nico, pela  presen&ccedil;a da &uacute;lcera com bordas elevadas e enduradas (borda  em moldura), presen&ccedil;a de granula&ccedil;&otilde;es grosseiras em fundo da les&atilde;o, associados a  dados epidemiol&oacute;gicos<sup>27</sup>. &Eacute; confirmado por meio de IDRM e exames parasitol&oacute;gico  direto e  histopatol&oacute;gico da les&atilde;o. Esses exames s&atilde;o os recomendados pelo MS para  aplica&ccedil;&atilde;o do crit&eacute;rio laboratorial.</font></p>     <p><font size="2" face="Verdana">O IDRM  avalia a resposta de hipersensibilidade celular retardada<sup>28</sup> e  apresenta sensibilidade em torno de 90%<sup>3</sup>, por&eacute;m a especificidade &eacute;  baixa devido &agrave; ocorr&ecirc;ncia de rea&ccedil;&otilde;es cruzadas<sup>29</sup>. Percebe-se que no &uacute;ltimo ano  estudado a utiliza&ccedil;&atilde;o do IDRM atingiu uma m&eacute;dia de 99,1% dos indicadores,  demonstrando a sua import&acirc;ncia na complementa&ccedil;&atilde;o do diagn&oacute;stico. Apesar da  baixa especificidade, Faber et al<sup>30</sup>, defendem sua utiliza&ccedil;&atilde;o  associada a crit&eacute;rios cl&iacute;nicos e recomenda sua aplica&ccedil;&atilde;o para o in&iacute;cio da  terap&ecirc;utica.</font></p>     <p><font size="2" face="Verdana">A  visualiza&ccedil;&atilde;o do parasito corado com Giemsa ao microsc&oacute;pio - exame parasitol&oacute;gico  direto - confirma  a LTA. Al&eacute;m de apresentar um baixo custo, &eacute; de simples e r&aacute;pida execu&ccedil;&atilde;o<sup>31</sup>.  Segundo o MS &eacute; o exame de primeira escolha para o diagn&oacute;stico de LTA<sup>11</sup>.  Mas observou-se que, em 10 anos,  sua utiliza&ccedil;&atilde;o foi pouco efetuada,  somente apresentando alguns picos nos anos de 2003 (33%), 2005 (29,7%) e 2006 (42,6%). Essa t&eacute;cnica tem sido  pouco utilizada em Alagoas pela falta de profissionais capacitados na  identifica&ccedil;&atilde;o dos amastigotas. Conforme Schwartz et al<sup>26</sup>, em um trabalho  de revis&atilde;o encontrou-se  de 19-77% de sensibilidade para o m&eacute;todo. Vega-Lop&eacute;z<sup>29</sup>,  em artigo de opini&atilde;o, fez refer&ecirc;ncia &agrave; sensibilidade de 50-70%. Andrade et al<sup>32</sup>,  em Pernambuco, para <i>L. braziliensis, </i>obteve-se uma sensibilidade de  47,9%. Como a prov&aacute;vel esp&eacute;cie respons&aacute;vel pela LTA &eacute; a <i>L. braziliensis, </i>esta  t&eacute;cnica &eacute; de baixa sensibilidade contribuindo para isso o fato de que muitos  pacientes apresentam les&otilde;es antigas, al&eacute;m do que este exame depende do tamanho  da amostra retirada<sup>33</sup> e da quantidade de parasitos na les&atilde;o.</font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana">O emprego das bi&oacute;psias de les&atilde;o foi bastante parecido  com o registrado nas pesquisas diretas do parasito, como se observa na <a href="#f3">figura 3</a>. A &uacute;nica diferen&ccedil;a  significativa ocorreu no ano de 2002 (56,8%) com um aumento da m&eacute;dia, devido a  uma pesquisa realizada para uma tese de doutorado utilizando a t&eacute;cnica de  imunohistoqu&iacute;mica. Esse m&eacute;todo diagn&oacute;stico s&oacute; &eacute; conclusivo quando detectada a  presen&ccedil;a dos amastigotas ou as caracter&iacute;sticas histol&oacute;gicas s&atilde;o compat&iacute;veis com  LTA, sen&atilde;o conclui-se como  sugestivo para LTA<sup>33</sup>. Nele se observa a presen&ccedil;a do granuloma  epiteli&oacute;de com infiltrado histio-plasmocit&aacute;rio e presen&ccedil;a de parasitos, por  vezes<sup>34</sup>. O estudo histopatol&oacute;gico das les&otilde;es &eacute; utilizado para  aprofundar o diagn&oacute;stico da LTA ou quando se necessita de dados para estudos de  perfis epidemiol&oacute;gicos, pois a detec&ccedil;&atilde;o do parasito varia em torno de 20% a 25%<sup>35</sup>  determinando uma baixa sensibilidade. Al&eacute;m disso, existe a dificuldade na obten&ccedil;&atilde;o  do exame, visto que este demanda uma an&aacute;lise em centros com experi&ecirc;ncia no  diagn&oacute;stico de LTA<sup>32</sup>.</font></p>     <p>&nbsp;</p>     <p><font size="3" face="Verdana"><b>CONCLUS&Atilde;O</b></font></p>     <p><font size="2" face="Verdana">Alagoas  apresenta um coeficiente de detec&ccedil;&atilde;o de n&iacute;vel m&eacute;dio (4,6 casos por 100 mil habitantes), de  acordo com os padr&otilde;es estabelecidos pelo manual de leishmaniose tegumentar  americana do MS. Esses dados evidenciaram a predomin&acirc;ncia de casos no sexo  masculino, de idade maior de 10 anos e com a forma cut&acirc;nea, refletindo no Estado o padr&atilde;o  nacional da doen&ccedil;a.</font></p>     <p><font size="2" face="Verdana">Esse  panorama exibe uma tend&ecirc;ncia de diminui&ccedil;&atilde;o no n&uacute;mero de casos ao longo dos anos  do estudo, todavia mostra um aumento na propor&ccedil;&atilde;o do sexo masculino na idade  maior ou igual a10 anos e nas les&otilde;es cut&acirc;neas.</font></p>     <p><font size="2" face="Verdana">O  presente estudo permitiu o conhecimento dos principais indicadores  epidemiol&oacute;gicos propostos pelo MS e que raramente s&atilde;o calculados pelas  vigil&acirc;ncias epidemiol&oacute;gicas. A constru&ccedil;&atilde;o das figuras de tend&ecirc;ncia, a partir da  regress&atilde;o linear, permite que os gestores possam prever a din&acirc;mica da endemia  para a tomada de decis&otilde;es.
