As antigas Colônia do Prata, em Igarapé-Açu, e Colônia Marituba, na região metropolitana de Belém, ambas no Pará, onde eram hospedados pacientes portadores do mal de Hansen, retratam uma realidade histórica da época em que os pacientes eram segregados e separados das respectivas famílias em função do contágio a que estavam expostas. O advento do tratamento multidroga ou poliquimioterápico, no Brasil na década de 1980, mudou consideravelmente o quadro da endemia no País, com uma redução acentuada da prevalência da doença; embora o aparecimento de casos novos não apresente redução significativa nos países endêmicos, ainda assim, o quadro epidemiológico sofreu mudanças substanciais nos últimos 20 anos. O Instituto Evandro Chagas (IEC), órgão de pesquisa da Secretaria de Vigilância em Saúde, Ministério da Saúde, iniciou estudos epidemiológicos e realizou atividades de vigilância em saúde pública referentes à hanseníase na década de 1990, mais precisamente, em 1998, quando aplicou um método laboratorial simples, denominado ML Dipstick, o qual pesquisa anticorpos da classe IgM contra o glicolipídeo fenólico-I (PGL-I), um antígeno de parede celular específico do Mycobacterium leprae, em cinco municípios endêmicos para hanseníase no Estado do Pará. Este estudo de revisão tem como objetivo descrever a trajetória do desenvolvimento do Programa de Hanseníase no IEC, bem como avanços e perspectivas futuras, relevantes no contexto da endemia hansênica no Pará.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[ABSTRACT Hanseniasis is a prevalent disease in Pará State, Brazil. The old settlements Colônia do Prata (Igarapé-Açu) and Colônia Marituba (metropolitan area of Belém), both in Pará, were home to patients with hanseniasis and portray a historical reality of patient segregation and separation from their families because of their contagious disease. Multidrug treatment (polychemotherapy) was established in Brazil during the 1980s and led to a significant countrywide decrease in the prevalence of hanseniasis. Although there were no significant decreases in the number of new cases reported in endemic countries, the epidemiologic framework dramatically changed during the past 20 years. Instituto Evandro Chagas (IEC) is a research agency of the Health Surveillance Secretariat of the Ministry of Health, which conducted epidemiologic studies and performed public health surveillance on hanseniasis during the 1990s. In 1998, a simple laboratory method called ML Dipstick was used for identifying IgM antibodies against the phenolic glycolipid-I (PGL-I), a Mycobacterium leprae-specific antigen; and those studies were conducted in five endemic municipalities located in Pará State. This report aimed to describe IEC's Hanseniasis Program, as well as provide future perspectives on hanseniasis in Pará State.]]></p></abstract>
<abstract abstract-type="short" xml:lang="es"><p><![CDATA[RESUMEN La lepra hace parte de las enfermedades prevalentes en el Estado de Pará, Brasil. Las antiguas Colônia do Prata, en Igarapé-Açu, y Colônia Marituba, Región Metropolitana de Belém, ambas de Pará, en donde eran hospedados pacientes portadores del mal de Hansen, retratan una realidad histórica de la época en que los pacientes eran segregados y separados de las respectivas familias en función del contagio a que estaban expuestas. El surgimiento del tratamiento multidroga, o poliquimioterápico, en Brasil en la década de 1980, cambió considerablemente el cuadro de la endemia en el País, ocasionando una reducción acentuada de la prevalencia de la enfermedad; aunque el aparecimiento de casos nuevos no presente una reducción significativa en los países endémicos, aun así, el cuadro epidemiológico sufrió cambios sustanciales en los últimos 20 años. El Instituto Evandro Chagas (IEC), órgano de investigación de la Secretaría de Vigilancia en Salud del Ministerio de Salud, inició estudios epidemiológicos y realizó actividades de vigilancia en salud pública referentes a la lepra en la década de 1990, más precisamente, en 1998, cuando aplicó un método de laboratorio simple, denominado "Ml Dipstick", el que analiza anticuerpos de la clase IgM contra el glicolípido fenólico-I (PGL-I), un antígeno de pared celular específico del Mycobcaterium leprae, en cinco municipios endémicos para lepra en el Estado de Pará. Este estudio de revisión tiene como objetivo describir la trayectoria del desarrollo del Programa de Lepra en el IEC, bien como los avances y perspectivas futuras, relevantes en el contexto de la endemia hansénica en Pará.]]></p></abstract>
