É causada por fungos demáceos presentes no solo, em vegetais em decomposição, plantas, flores e madeira.  OBJETIVO:  Descrever um caso de CBM de evolução prolongada e lesão extensa, destacando a importância do manejo adequado da doença.  DESCRIÇÃO DO CASO:  D.M.S., sexo masculino, 56 anos, trabalhador rural, residente no estado do Pará. Iniciou com lesão no membro inferior direito, inicialmente manifestada como pequena pápula com sinais flogísticos, que evoluiu, após alguns dias, para lesão verrucosa. O paciente foi atendido por diferentes profissionais e recebeu tratamentos diversos com pomadas dermatológicas, sem resolução dos sintomas.  RESULTADOS:  Foi realizado exame micológico direto, com resultado negativo para fungos. Posteriormente, procedeu-se à exérese de lesão vegetante, que foi encaminhada para biópsia. O exame histopatológico identificou achados compatíveis com CBM. Iniciou-se tratamento com itraconazol (duas cápsulas de 100 mg, por via oral, duas vezes ao dia, por 12 meses). Ao final do tratamento, houve regressão completa das lesões e dos sintomas associados, permanecendo apenas lesões maculares residuais.  CONCLUSÃO:  A CBM é uma doença negligenciada, e o atraso no diagnóstico pode gerar prejuízos significativos à vida dos indivíduos acometidos por essa infecção.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[ABSTRACT  INTRODUCTION:  Chromoblastomycosis (CBM) is a chronic, slowly progressing fungal infection with a polymorphic appearance that affects the cutaneous and subcutaneous tissues. It is caused by dematiaceous fungi found in soil, decomposing vegetation, plants, flowers, and wood.  OBJECTIVE:  To describe a case of CBM with chronic evolution and extensive lesion, emphasizing the importance of appropriate disease management.  CASE DESCRIPTION:  D.M.S., male, 56 years old, rural worker, resident in the state of Pará. He initially presented with a lesion on the right lower limb, which began as a small papule with inflammatory signs and progressed, within a few days, to a verrucous lesion. The patient was seen by several healthcare professionals and received various dermatological treatments, with no resolution of symptoms.  RESULTS:  A direct mycological examination was performed, with negative results for fungal structures. Subsequently, a vegetative lesion was excised and sent for biopsy. Histopathological examination revealed findings compatible with CBM. Treatment with itraconazole was initiated (two 100 mg capsules, taken orally twice daily for 12 months). At the end of treatment, the lesions had completely regressed, as had the associated symptoms, although residual macular lesions were still present.  CONCLUSION:  CBM is a neglected disease, and delays in diagnosis may lead to significant harm to affected individuals.]]></p></abstract>
